地黄益智汤联合西医治疗血管性帕金森综合征的疗效观察

摘　　要：目的观察地黄益智汤联合西医治疗血管性帕金森综合征(VP)的临床疗效。方法将80例VP患者根据治疗方法分为2组,对照组36例予多巴丝肼片联合重复脉冲经颅磁刺激(rTMS)治疗,治疗组44例在对照组治疗基础上加用地黄益智汤治疗。2组均治疗6周后统计疗效,比较2组治疗前后统一帕金森综合量表(UPDRS)评分、自主神经量表(SCOPA-AUT)评分、蒙特利尔认知评估量表(MoCA)评分、Barthel指数评定量表(BI)评分变化情况,比较2组治疗前后血清谷胱甘肽(GSH)、一氧化氮(NO)、同型半胱氨酸(Hcy)水平变化情况。结果治疗组总有效率95.45%(42/44),对照组为80.56%(29/36),治疗组疗效优于对照组(P<0.05)。与本组治疗前比较,2组治疗后UPDRS评分、SCOPA-AUT评分均降低(P<0.05),MoCA评分、BI评分均升高(P<0.05),且治疗组治疗后改善均优于对照组(P<0.05)。与本组治疗前比较,2组治疗后血清GSH、NO水平均升高(P<0.05),Hcy水平均降低(P<0.05),且治疗组改善均优于对照组(P<0.05)。结论地黄益智汤联合西医治疗VP临床疗效确切,可明显改善患者临床症状、体征,改善自主神经功能障碍和认知功能障碍,提高日常生活能力,升高血清GSH、NO水平,降低Hcy水平。Objective To study the curative effect of Dihuang Yizhi Decoction combined with Western medicine on vascular parkinsonism(VaP)after stroke.Methods A total of 80 VaP patients were enrolled and divided into combination group(44 cases)and control group(36 cases).On basis of Conventional treatment(Dobutazide tablets with repetitive transcranial magnetic stimulation),the combination group was additionally treated with Dihuang Yizhi Decoction.A 6-week continuous treatment was performed to assess the changes of the change of Unified Parkinson's Disease Rating Scale(MDS-UPDRS),Parkinson's Disease-Autonomic(SCOPA-AUT),Montreal Cognitive Assessment(MoCA),Barthel Index(BI)rating scale,Glutathione(GSH),nitric oxide(NO)and homocysteine(Hcy)levels were calculated before and after treatment.Results The response rate of combination group was higher than that of control group(95.45%[42/44]vs 80.56%[29/36])(P<0.05).Compared with before treatment,down-regulated UPDRS and SCOPA-AUT score,and up-regulated MoCA and BI score were detected in groups after treatment(P<0.05),and the improvement was better in the combination group relevant to the control group(all P<0.05).Increased GSH and NO levels and decreased Hcy were found in both groups after treatment(all P<0.05),and which in the combination group was superior to the control group(P<0.05).Conclusion Dihuang Yizhi Decoction combined with Western medicine can obviously relieve the clinical symptoms and physical signs of VaP patients,thereby,improving autonomic nerve dysfunction and cognitive dysfunction,improving daily living ability,and increasing serum GSH and NO levels.
