气海俞龙虎交战针法治疗腰椎间盘突出症临床研究

摘　　要：目的观察气海俞龙虎交战针法治疗腰椎间盘突出症(LDH)的临床疗效。方法将60例LDH患者按照随机数字表法分为2组,治疗组30例予气海俞龙虎交战针法治疗,对照组30例予常规针刺治疗。2组均治疗2个疗程后统计疗效,比较2组治疗前后疼痛视觉模拟评分(VAS)、日本骨科协会评估治疗分数(JOA)下腰痛评分变化情况,比较2组肌电图指标双侧腓总神经和胫神经的运动神经传导速度和远端潜伏期变化情况。结果治疗组总有效率93.3%(28/30),对照组总有效率83.3%(25/30),治疗组疗效优于对照组(P<0.05);与本组治疗前比较,2组治疗后疼痛VAS均降低(P<0.05),JOA下腰痛评分均升高(P<0.05),且治疗组治疗后改善均优于对照组(P<0.05)。与本组治疗前比较,2组治疗后双侧腓总神经和胫神经的运动神经传导速度均增快(P<0.05),远端潜伏期均缩短(P<0.05),且治疗组治疗后改善均优于对照组(P<0.05)。结论气海俞龙虎交战针法治疗LDH临床疗效确切,可明显缓解患者疼痛症状,改善腰椎功能,促进受损的神经修复。Objective To observe the clinical effect of Qihaishu(BL24)"Dragon-Tiger Fighting"acupuncture for lumbar disc herniation(LDH).Methods Totally 60 patients with LDH were randomly divided into treatment group(n=30,received Qihaishu[BL24]"Dragon-Tiger Fighting"acupuncture)and control group(n=30,received conventional acupuncture).The treatment continued for 2 courses,visual analogue score(VAS),japanese orthopaedic association(JOA)score for low back pain,electromyogram indexes(motor nerve conduction velocity[MNCV]and motor nerve distal latency[MNDL]of bilateral common peroneal nerve[CPN]and tibial nerve)were included as comparators.Results The overall effective rate in treatment group was better than that in control group(93.3%[28/30]vs 83.3%[25/30],[P<0.05],respectively).After treatment,VAS in groups was decreased,and JOA score for low back pain was increased(P<0.05),improvement of which in treatment group were significantly better(P<0.05);MNCV of bilateral CPN and tibial nerve in groups was increased(P<0.05),while MNDL was decreased(P<0.05),which in treatment group were more common(P<0.05).Conclusion Qihaishu(BL24)"Dragon-Tiger Fighting"acupuncture for LDH patients has a definite clinical effect,which is obviously capable of relieving pain symptoms,improving lumbar function,and promoting repair of injured nerve.
