陈旧性下胫腓联合韧带损伤临床治疗标准化研究

摘　　要：目的:探讨采用免打结锁扣带袢钢板(Knotless Tightrope)方式对陈旧性踝关节骨折合并下胫腓联合韧带损伤患者进行治疗,观察其临床疗效。方法:前瞻性研究云南省绿春县人民医院自2015年1月至2020年6月收治的36例陈旧性踝关节骨折合并下胫腓联合韧带损伤患者,所有采用Knotless Tightrope治疗,术后使用美国矫形足踝协会(AOFAS)踝-后足评分标准及足踝功能测试(FAAM)评分以评价疗效。结果:手术后随访12~18个月,AOFAS评分及FAAM评分均高于治疗前(P<0.05),临床疗效优28例,良8例。结论:Knotless Tightrope治疗陈旧性踝关节骨折合并下胫腓联合韧带损伤具有较好的临床疗效,但无对比分析,需在以后研究中进一步探讨。Objective:To investigate the clinical effect of knotless tightrope in the treatment of old ankle fracture complicated with inferior tibiofibular ligament injury.Methods:36 patients with old ankle fracture complicated with inferior tibiofibular ligament injury treated in Yunnan Luchun County People’s Hospital from January 2015 to June2020 were prospectively studied.All patients were treated with knotless tightrope.The ankle hindfoot score standard of American Orthopaedic Foot and Ankle Association(AOFAS)and Foot and Ankle Ability Measure(FAAM)were used to evaluate the curative effect.Results:After follow-up for 12~18 months,AOFAS score and FAAM score were higher than those before treatment(P<0.05).The clinical curative effect was excellent in 28 cases and good in 8 cases.Conclusion:The treatment of old ankle fracture complicated with inferior tibiofibular ligament injury with knotless tightrope has good clinical effect,and the comparative analysis need to be further analyzed in future research.
