摘 要:分析妊娠期高血压患者血清表皮生长因子(EGF)、神经降压素(NT)表达水平,并探讨EGF、NT与子痫前期的相关性。选取2017年1月—2020年6月在邯郸市中心医院就诊的88例妊娠期高血压患者为研究对象,其中单纯妊娠高血压患者49例(妊娠高血压组),子痫前期患者39例(子痫前期组);依据子痫前期轻重度诊断标准将子痫前期患者分为子痫前期轻度患者27例(子痫前期轻度组),子痫前期重度患者12例(子痫前期重度组);同时期选取邯郸市中心医院收治的正常妊娠孕妇88例作为健康妊娠组。采用酶联免疫吸附(ELISA)法检测血清中EGF、NT及血管收缩因子神经肽Y(NPY)水平;采用受试者工作特征(ROC)曲线分析血清EGF、NT及二者联合对子痫前期发生的预测价值。结果显示:与健康妊娠组相比,妊娠期高血压组、子痫前期组舒张压、收缩压、身体质量指数(BMI)及血清NPY水平较高(P<0.05),EGF、NT水平较低(P<0.05);与妊娠期高血压组相比,子痫前期组患者血清EGF、NT水平较低(P<0.05),NPY水平较高(P<0.05);与子痫前期轻度组相比,子痫前期重度组患者血清EGF、NT水平较低(P<0.05),NPY水平较高(P<0.05);ROC曲线显示,EGF对子痫前期发生的曲线下面积(AUC)为0.799,其灵敏度、特异度分别为67.30%、82.10%;NT对子痫前期发生的AUC为0.800,其灵敏度、特异度分别为71.40%、84.60%;二者联合对子痫前期发生的AUC为0.862(95%CI:0.780~0.945),其灵敏度、特异度分别为89.80%、82.10%。研究表明妊娠期高血压患者血清EGF、NT呈低表达,且与子痫前期的发生发展具有密切关系。The purpose of this paper is to analyze the expression levels of serum epidermal growth factor(EGF)and neurotensin(NT)in patients with pregnancy induced hypertension(PIH),and to explore the correlation between EGF,NT and preeclampsia.From January 2017 to June 2020,88 patients with gestational hypertension in Han Dan Central Hospital were selected as the research objects,including 49 cases of pregnancy induced hypertension(PIH group)and 39 cases of preeclampsia(preeclampsia group).According to the diagnostic criteria of mild and severe preeclampsia,the preeclampsia patients were divided into 27 cases of mild preeclampsia patients(mild preeclampsia group),and 12 cases of severe preeclampsia patients(severe preeclampsia group);at the same time,88 cases of normal pregnant women in Han Dan Central Hospital were selected as healthy pregnancy group.Enzyme linked immunosorbent assay(ELISA)was used to detect the levels of EGF,NT and vasoconstrictor neuropeptide Y(NPY);receiver operating curve(ROC)was used to analyze the predictive value of serum EGF,NT and their combination for preeclampsia.Compared with those in the healthy pregnancy group,the levels of diastolic blood pressure,systolic blood pressure,body mass index(BMI)and serum NPY level were higher in hypertensive pregnancy group and preeclampsia group(P<0.05),while the levels of EGF and NT were lower(P<0.05).Compared with those in the pregnancy induced hypertension group,the levels of serum EGF and NT in preeclampsia group were lower(P<0.05),and the level of NPY was higher(P<0.05).Compared with those in mild preeclampsia group,the levels of serum EGF and NT in severe preeclampsia group were lower(P<0.05),and the level of NPY was higher(P<0.05).ROC curve showed that the area under the cure(AUC)of EGF was 0.799,and its sensitivity and specificity were 67.30%and 82.10%,respectively;the AUC of NT for preeclampsia was 0.800,and its sensitivity and specificity were 71.40%and 84.60%,respectively;the AUC of the combination of the two for the occurrence of preeclampsia was 0
