醒脑开窍针刺法对急性脑梗死患者炎性反应及血液循环的影响

摘　　要：目的:探讨醒脑开窍针刺法对急性脑梗死患者的临床疗效及对炎性反应和血液循环的影响。方法:选取符合纳入标准的108例急性脑梗死患者,按照类层抽样法分为对照组和观察组,各54例。对照组予以胞二磷胆碱及阿替普酶治疗,观察组予以醒脑开窍针刺联合胞二磷胆碱及阿替普酶治疗,两组均治疗4周。治疗结束后对比两组患者临床总有效率;观察治疗前后的C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、脑血容量(CBV)、血流峰值时间(TTP)、平均血流量(Qmean)及阻力指数(RI)的变化情况。结果:观察组总有效率(96.3%,52/54)高于对照组(74.07%,40/54),差异具有统计学意义(P <0.05)。治疗前两组患者CRP、TNF-α、IL-6、CBV、TTP、Qmean、RI水平比较,差异无统计学意义(P> 0.05);治疗4周后,两组各观察指标均得到改善(P <0.05),且观察组的CRP、TNF-α、IL-6、TTP、RI均低于对照组(P <0.01,P <0.05),CBV、Qmean高于对照组(P <0.01)。结论:醒脑开窍针刺联合胞二磷胆碱及阿替普酶治疗急性脑梗死疗效显著,有利于降低患者机体的炎性反应,并改善血液循环。Objective:To explore the clinical effect of Xingnao Kaiqiao acupuncture on acute cerebral infarction and its influence to inflammatory reaction and blood circulation.Methods:108 patients with acute cerebral infarction were randomly divided into the control group and the observation group according to the stratified sampling method,with 54 patients in each group.The control group was treated with Citicoline and Alteplase;on which basis,the observation group was also treated with Xingnao Kaiqiao acupuncture.The total clinical effective rate was compared between the two groups.The changes of C-reactive protein(CRP),tumor necrosis factor(TNF-α),interleukin-6(IL-6),cerebral blood volume(CBV),time to peak blood flow(TTP),average blood flow(Qmean)and resistance index(RI)were observed before and after the treatment in the two groups.Results:The total effective rate was 96.3%(52/54)in the observation group,which was significantly higher than74.07%(40/54)in the control group(P < 0.05).There were no statistical differences in the levels of CRP,TNF-α,IL-6,CBV,TTP,Qmean and RI between the two groups before the treatment(P > 0.05);which were significantly improved after the treatment in the two groups(P < 0.05);the levels of CRP,TNF-α,IL-6,TTP and RI were significantly lower(P < 0.05,P < 0.01),and the levels of CBV and Qmean were greatly higher in the observation group than those in the control group(P < 0.01).Conclusion:Xingnao Kaiqiao acupuncture combined with Citicoline and Alteplase has a significant effect on acute cerebral infarction,which is beneficial to reduce inflammation and improve blood circulation.
