H型高血压与冠心病病变严重程度的相关性研究

摘　　要：目的探讨H型高血压与冠心病病变严重程度的相关性。方法选取2019年1月至2021年1月郑州市第二人民医院90例高血压合并冠心病患者作为研究对象,根据血清同型半胱氨酸水平(Hcy)将患者分为H型高血压组(n=50)与非H型高血压组(n=40)。比较两组血压、血脂及血尿酸等水平,探讨H型高血压与冠心病病变严重程度的相关性。结果H型高血压组与非H型高血压组年龄、性别、血压、血脂等各项指标比较,差异未见统计学意义(P>0.05),但H型高血压组Hcy水平更高,差异有统计学意义(P<0.05)。H型高血压组冠状动脉轻度狭窄低于非H型高血压组,重度狭窄高于非H型高血压组,差异有统计学意义(P<0.05)。H型高血压组Gensini积分高于非H型高血压组,差异有统计学意义(P<0.05)。Spearman相关性分析显示H型高血压与冠心病病变严重程度呈正相关(r=0.719,P<0.05)。结论H型高血压与冠心病病变严重程度密切相关,应加强对高血压患者Hcy水平的监测,预测冠心病风险,积极控制高血压,改善预后。Objective To investigate the relatinship between H-type hypertension and the severity of coronary heart disease.Methods Ninty patients with hypertension complicated with coronary heart disease in Zhengzhou Second People’s Hospital from January 2019 to January 2021 were selected as the research objects.According to the level of serum homocysteine(Hcy),the patients were divided into H-type hypertension group(n=50)and non-H-type hypertension group(n=40).The levels of blood pressure,blood lipid and blood uric acid in the two groups were compared,and the relationship between H-type hypertension and the severity of coronary heart disease was investigated.Results There was no significant difference in age,sex,blood pressure or blood lipid between the two groups(P>0.05),but the level of Hcy in the H-type hypertension group was higher than that in non-H-type hypertension group(P<0.05).Mild coronary artery stenosis in H-type hypertension group was lower than that in non-h-type hypertension group,and severe coronary artery stenosis was higher than that in non-h-type hypertension group(P<0.05).The Gensini score in H-type hypertension group was significantly higher than that in non-H-type hypertension group(P<0.05).Spearman correlation analysis showed that H-type hypertension was positively correlated with the severity of coronary heart disease(r=0.719,P<0.05).Conclusions H-type hypertension is closely correlated with the severity of coronary heart disease.We should strengthen the monitoring of Hcy level in patients with hypertension,predict the risk of coronary heart disease,actively control hypertension and improve the prognosis.
