利司那肽联合甘精胰岛素治疗口服药控制不佳的2型糖尿病的效果

摘　　要：目的研究口服药控制不佳的2型糖尿病患者应用利司那肽联合甘精胰岛素对比预混胰岛素的疗效。方法选取2019年9月至2020年1月于河南省人民医院内分泌科收治的口服3种或以上降糖药物3个月后,血糖仍未达标[糖化血红蛋白(HbA1c)7%~11%]的2型糖尿病患者62例,应用随机数字表法分为观察组与对照组,每组31例。观察组应用利司那肽注射液联合甘精胰岛素治疗,对照组给予门冬胰岛素30注射液治疗,比较两组治疗效果。结果两组治疗前空腹血糖(FBG)及三餐餐后2 h血糖(2 h PG)、HbA1c、空腹C肽(FCP)、餐后2h C肽(2h PCP)、体质指数(BMI)比较,差异未见统计学意义(P>0.05)。治疗后,观察组2h PG、HbA1c、BMI均低于对照组,差异有统计学意义(P<0.05);观察组低血糖发生率显著低于对照组,差异有统计学意义(χ^(2)=5.415,P=0.020)。结论利司那肽联合甘精胰岛素与同样每天2次皮下注射的预混胰岛素方案比较,改善血糖达标的同时能实现更少的低血糖发生率和良好的体质量控制等多重获益。Objective To study the efficacy of risenatide combined with insulin glargine compared with premixed insulin on patients with type 2 diabetes who are poorly controlled by oral drugs.Methods A total of 62 patients with type 2 diabetes mellitus(HbA1c)7%to 11%]were enrolled in the Department of Endocrinology,Henan Province People’s Hospital from September 2019 to January 2020.After oral administration of 3 or more hypoglycemic drugs for 3 months,the patients were divided into the observation group and the control group,with 31 cases in each group.The observation group was treated with lisnatide injection combined with insulin glargine,and the control group was treated with insulin aspart 30 injection.The therapeutic effects of the two groups were compared.Results There was no significant difference in FBG,2 h PG,HbA1c,FCP,2 h PCP,BMI between the two groups before treatment(P>0.05).After treatment,the 2 h PG,HbA1c,and BMI of the experimental group were lower than those of the control group,and the difference was significant(P<0.05).The incidence of hypoglycemia in the experimental group was significantly lower than that in the control group,and the differences were significant(χ^(2)=5.415,P=0.020).Conclusions Compared with the premixed insulin regimen of the same subcutaneous injection twice a day,the combination of risnatide and insulin glargine can improve the blood glucose standard while achieving multiple benefits such as less incidence of hypoglycemia and good weight control.
