滋膵通脉饮联合甲钴胺治疗糖尿病周围神经病变30例临床观察

摘　　要：目的:观察滋膵通脉饮联合甲钴胺治疗糖尿病周围神经病变(DPN)气阴两虚、瘀血阻络证的临床疗效。方法:将60例DPN气阴两虚、瘀血阻络证患者随机分为治疗组与对照组,每组各30例。对照组予以西医常规治疗,治疗组在对照组基础上加用滋膵通脉饮治疗。观察2组治疗前后多伦多临床评分系统(TCSS)评分、感觉阈值(VPT)、正中神经(运动神经、感觉神经)传导速度的变化,并评价综合疗效。结果:总有效率治疗组为73.33%(22/30),高于对照组的63.33%(19/30),差异有统计学意义(P<0.05)。2组TCSS评分、VPT、正中神经传导速度治疗前后差值及治疗后组间比较,差异均有统计学意义(P<0.05或P<0.01)。结论:滋膵通脉饮联合甲钴胺治疗DPN气阴两虚、瘀血阻络证临床疗效显著,可缓解患者疼痛、麻木等症状,提高神经传导速度,改善患者生活质量。Objective: To investigate the clinical effect of Zicui Tongmai decoction combined with mecobalamin in the treatment of diabetic peripheral neuropathy(DPN) with deficiency of both Qi and Yin and blood stasis obstructing the collaterals.Methods: A total of 60 patients with DPN with deficiency of both Qi and Yin and blood stasis obstructing the collateralswere randomly divided into treatment group and control group, with 30 patients in each group.The patients in the control group were given conventional Western medicine treatment, and those in the treatment group were given Zicui Tongmai decoction in addition to the treatment in the control group.The two groups were observed in terms of the changes in Toronto clinical scoring system(TCSS) score, sensory threshold(VPT),and median nerve(motor nerve and sensory nerve) conduction velocity after treatment, and overall response was evaluated for both groups.Results: The treatment group had a significantly higher overall response rate than the control group [73.33%(22/30)vs 63.33%(19/30),P<0.05].After treatment, both groups had significant changes in TCSS score, VPT,and median nerve conduction velocity, and there were significant differences in these indices between the two groups after treatment(P<0.05 or P<0.01).Conclusion: Zicui Tongmai decoction combined with mecobalamin has a marked clinical effect in the treatment of DPN with deficiency of both Qi and Yin and blood stasis obstructing the collateralsand can alleviate the symptoms of pain and numbness, increase nerve conduction velocity, and improve patients’ quality of life.
