摘  要:目的探究类胡萝卜素影响胰岛素抵抗产妇脐带内皮细胞内源性性激素结合球蛋白(SHBG)水平与葡萄糖转运蛋白(GLUT)的机制。方法回顾性选取2019年1月至2021年6月在佛山市高明区人民医院诊断为妊娠糖尿病(GDM)的患者60例,根据干预方式不同分为预防组和病例组,每组各30例。预防组在糖尿病常规治疗基础上定期服用类胡罗素进行预防治疗,病例组仅给予糖尿病常规治疗。另选取同时期在该院做产检的30名糖代谢水平正常的妊娠孕妇作为对照组。采用蛋白质印迹法分析胎盘组织中SHBG、葡萄糖相关转运蛋白(GLUT)-3、GLUT-4的表达。采用PCR实时分析胰岛素信号转导蛋白IRS-1、IRS-2、PI3K、p85α的mRNA表达。血常规检查检测血清SHBG及相关激素[雌二醇、促黄体生成激素(LH)及游离睾丸激素(T)]水平。结果病例组SHBG的蛋白表达低于对照组,预防组SHBG的蛋白表达高于病例组,差异均有统计学意义(P<0.05)。病例组IRS-1、IRS-2、PI3K mRNA表达均低于对照组,预防组的IRS-1、IRS-2、PI3K的mRNA表达均高于病例组,差异均有统计学意义(P<0.05),p85αmRNA各组表达差异无统计学意义(P>0.05)。病例组GLUT-3、GLUT-4的蛋白表达均低于对照组,预防组GLUT-3、GLUT-4的蛋白表达均高于病例组,差异均有统计学意义(P<0.05)。病例组的SHBG水平含量低于对照组,雌二醇、T、LH水平含量均高于对照组,预防组的SHBG水平含量高于病例组,雌二醇、T、LH水平低于病例组,差异均有统计学意义(P<0.05)。结论GDM孕妇胎盘中胰岛素信号转导和葡萄糖转运蛋白存在异常,而类胡罗素可调节相关蛋白表达,因此通过类胡萝卜素预防性治疗可促进孕妇SHBG的合成分泌,抑制胰岛素抵抗和GDM的发生。Objective To study the mechanism of carotenoids affecting endogenous sex hormone binding globulin(SHBG)levels and glucose transporter(GLUT)in umbilical cord endothelial cells of insulin-resistant mothers.Methods Sixty patients diagnosed with gestational diabetes mellitus(GDM)in People's Hospital of Gaoming District in Foshan City from January 2019 to June 2021 were retrospectively selected and divided into prevention group and case group according to different intervention methods,with 30 cases in each group.The prevention group was given carotenoids regularly on the basis of routine diabetes treatment,while the case group was only given routine diabetes treatment.In addition,30 pregnant women with normal glucose metabolism who underwent obstetric examination in the hospital during the same period were selected as the control group.The expressions of SHBG,glucose-related transporters GLUT-3 and GLUT-4 in placental tissues were analyzed by Western blotting.The expressions of insulin signal transduction proteins IRS-1,IRS-2,PI3K and p85αmRNA were analyzed by real-time PCR.The levels of serum SHBG and related hormones[estradiol,luteinizing hormone(LH),and free testosterone(T)]were detected by routine blood tests.Results The protein expression of SHBG in the case group was lower than that in the control group,and the protein expression of SHBG in the prevention group was higher than that in the case group,the differences were statistically significant(P<0.05).The expression of IRS-1,PI3K and IRS-2 mRNA in the case group were lower than those in the control group,the expression of IRS-1,IRS-2 and PI3K mRNA in the prevention group were higher than those in the case group,the differences were statistically significant(P<0.05);there was no statistically significant difference in the expression of p85αmRNA among the three groups(P>0.05).The expression of GLUT-3 and GLUT-4 protein in the case group were lower than those in the control group,and the expression of GLUT-3 and GLUT-4 protein in the prevention group were high
