达沙替尼致可逆性肺动脉高压1例并临床特征分析

摘　　要：达沙替尼导致肺动脉高压(pulmonary arterial hypertension,PAH)目前国内病例报道较少,本例为56岁男性慢性粒细胞白血病患者,接受达沙替尼(100 mg/d)维持治疗,47个月后出现活动后胸闷气促,超声心动图检查提示右房室增大,肺动脉收缩压117 mmHg,右心导管检查明确诊断毛细血管前性PAH,考虑达沙替尼导致PAH。停服达沙替尼并抗PAH治疗后病情得到明显改善,再次服用达沙替尼后肺动脉压又再次加重,停药并继续抗PAH治疗后病情又明显好转直至完全缓解,长期随访病情稳定未反复。There are few reports about pulmonary hypertension caused by dasatinib in China.In this case,a 56-year-old male patient with chronic myelogenous leukemia received dasatinib(100 mg/d)maintenance therapy.After 47 months,he developed chest tightness and shortness of breath after activity.Echocardiography revealed that the right atrium and ventricle was enlarged and the pulmonary artery systolic pressure was 117 mmHg.Right heart catheterization confirmed the diagnosis of precapillary pulmonary arterial hypertension,and dasatinib was considered to cause the pulmonary arterial hypertension.After stopping dasatinib,the condition was significantly improved after the anti-pulmonary hypertension treatment.Pulmonary artery pressure returned when the patient started taking dasatinib again.After stopping the drug and continuing the anti-pulmonary hypertension therapy,the patient's condition improved significantly until complete remission.The long-term follow-up condition is stable.
