布地奈德两种局部给药方式对嗜酸性粒细胞性鼻窦炎术后的疗效观察

摘　　要：目的研究布地奈德的2种鼻腔局部给药方式对嗜酸性粒细胞性鼻窦炎伴鼻息肉(ECRSwNP)轮廓化术后患者恢复的影响。方法将52例经过鼻内镜鼻腔鼻窦轮廓化手术治疗的ECRSwNP患者随机分为2组,A组予布地奈德混悬液加入到智能鼻腔护理器经鼻腔雾化吸入治疗,每次1 mg,qd,B组给予布地奈德鼻喷雾剂喷鼻治疗,每次每鼻孔喷入128μg,qd。2组患者治疗时间均为3个月。治疗前、后评估鼻部症状、内镜评分,以不良事件来评估其安全性。结果2组患者鼻部症状治疗后均显明改善(P<0.05)。治疗后,A组和B组鼻堵、嗅觉下降、头面部胀痛评分差异均无统计学意义(均P>0.05)。术后3个月,A组和B组的流涕评分分别为(1.75±1.03)和(2.92±1.23)分,鼻内镜评分分别为1.00(1.00,2.00)和2.00(1.00,2.25)分,差异均有统计学意义(均P<0.05)。A、B组均未发生明显不良事件。结论经鼻雾化吸入布地奈德混悬液能明显减轻嗜酸性粒细胞性鼻窦炎伴鼻息肉轮廓化术后术腔黏膜的充血水肿及黏液分泌,明显加快术后术腔黏膜上皮化进程,提高手术疗效,可作为ECRSwNP术后常规治疗方法。Objective To study the influence of two methods of local nasal administration of glucocorticoid budesonide on the postoperative recovery of sinus contour surgery of eosinophilic chronic nasosinusitis with nasal polyp(ECRSwNP).Methods Fifty-two patients with ECRSwNP were randomly divided into two groups after endoscopic sinus contour surgery,group A was treated with budesonide suspension added to the intelligent nasal nursing device for nasal atomization inhalation,1 mg a time,once a day,while group B was treated with budesonide nasal spray,128μg a time,once a day per nostril.The treatment time of both groups was 3 months.Before and after treatment,nasal symptoms and endoscopic scores were evaluated.Meanwhile,adverse events were used to evaluate their security.Results Patients in both groups showed obvious improvements in their nasal symptoms after treatment(P<0.05).After treatment,there were no significant differences between group A and group B in nasal blockage,olfactory decline,head and face pain scores(all P>0.05).Three months after surgery,the running nose scores in group A and B were(1.75±1.03)and(2.92±1.23),respectively,and the nasal endoscopic scores were 1.00(1.00,2.00)and 2.00(1.00,2.25),respectively;the differences were statistically significant(all P<0.05).There were no obvious adverse events in group A and B.Conclusion The inhalation of budesonide suspension via nasal atomization can obviously relieve symptoms of congestive edema and mucus secretion of nasal mucous membrane after sinus contour surgery of eosinophilic chronic nasosinusitis with nasal polyp,and speed up the epithelization of nasal mucous membrane after surgery to improve the effect of surgery,which can be used as a conventional treatment after the surgery of ECRSwNP.
