摘　　要：目的检测微小RNA-4491(miR-4491)在胃癌患者血清中的表达水平,分析其与胃癌临床病理特征的相关性,探讨其单独检测及其与CA72-4联合检测在胃癌诊断中的临床价值。方法实时荧光定量聚合酶链反应检测胃癌患者和健康受试者血清中miR-4491的表达水平,通过受试者工作特征(ROC)曲线、曲线下面积(AUC)、危险分数分析法评估miR-4491对胃癌的诊断效能,计算灵敏度、特异度、阳性预测值(PPV)和阴性预测值(NPV),探讨miR-4491独立及与CA72-4联合检测在胃癌诊断中的应用价值。结果胃癌组和健康对照组血清miR-4491的相对表达水平分别为3.65±4.04和1.29±1.21,二者比较,差异有统计学意义(P<0.01),miR-4491的表达量与胃癌临床分期、淋巴结及远处转移均不相关(P>0.05),但与其病理分化程度呈负相关(P<0.05)。miR-4491独立检测诊断胃癌的AUC为0.78,敏感度、特异度、PPV、NPV分别为84.5%,59.4%,74.0%和72.9%,而CA72-4诊断胃癌的AUC为0.71,敏感度、特异度、PPV和NPV分别为20.9%,78.3%,51.9%和47.0%。MiR-4491与CA72-4联合诊断的AUC为0.83,PPV和NPV分别提高至87.1%和89.7%。结论miR-4491在胃癌血清中高表达,是胃癌临床筛查的潜在血清学标志物。Objective To detect the expression level of microRNA-4491(miR-4491)in the serum of patients with gastric cancer,analyze its correlation with the clinicopathological features of gastric cancer,and explore the clinical value of its single and combined detection with CA72-4 in the diagnosis of gastric cancer.Methods The expression of serum mir-4491 in patients with gastric cancer and healthy subjects was detected by real-time fluorescence quantitative pllymerae chain reaction.The diagnostic efficacy of miR-4491 for gastric cancer was evaluated by receiver operating characteristic(ROC)curve,area under curve(AUC)and risk score analysis.To explore the application value of miR-4491 independent and combined with CA72-4 in the diagnosis of gastric cancer by calculating sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV).Results The relative expression levels of serum miR-4491 in gastric cancer group and healthy control group were 3.65±4.04 and 1.29±1.21 and the difference was statistically significant(P<0.01).The expression of miR-4491 was not correlated with clinical stage,lymph nodes and distant metastasis of gastric cancer(P>0.05),but negatively correlated with the degree of pathological differentiation(P<0.05).The AUC of miR-4491 in the diagnosis of gastric cancer was 0.78,and the sensitivity,specificity,PPV and NPV were 84.5%,59.4%,74.0%and 72.9%respectively,while the AUC of CA72-4 in the diagnosis of gastric cancer was 0.71,and the sensitivity,specificity,PPV and NPV were 20.9%,78.3%,51.9%and 47.0%respectively.The AUC of miR-4491 combined with CA72-4 was 0.83,and the PPV and NPV were raised to 87.1%and 89.7%,respectively.Conclusion MiR-4491 is highly expressed in the serum of gastric cancer and is a potential serological marker for clinical screening of gastric cancer.
