GnRH-a联合LNG-IUS治疗子宫腺肌症的近远期疗效分析

摘　　要：目的 分析促性腺激素释放剂(GnRH-a)联合左炔诺孕酮宫内缓释系统(LNG-IUS)治疗子宫腺肌症的近远期疗效。方法 随机选取该院2018年12月—2020年11月接受治疗的60例子宫腺肌症患者,采用随机数表法将患者分为观察组(n=30)和对照组(n=30),对照组接受LNG-IUS治疗,观察组加用GnRH-a进行治疗;比较两组患者近远期疗效、月经量、痛经评分、子宫体积、血红蛋白变化。结果 观察组患者治疗6、12个月后治疗总有效率(93.33%、86.21%)高于对照组,差异有统计学意义(χ^(2)=3.456、4.028,P<0.05)。治疗6、12个月观察组患者月经量明显低于对照组,差异有统计学意义(t=2.763、3.426,P<0.05);治疗6、12个月后观察组患者痛经程度明显低于对照组,差异有统计学意义(t=3.913、3.790,P<0.05);治疗6、12个月后观察组患者子宫体积明显低于对照组,差异有统计学意义(t=4.032、9.940,P<0.05);治疗6、12个月后观察组患者血红蛋白水平明显高于对照组,差异有统计学意义(t=6.490、7.952,P<0.05)。结论 Gn RH-a联合LNG-IUS治疗子宫腺肌患者较单独应用LNG-IUS可明显提高患者近远期疗效,维持经量正常,减轻痛经程度,缩小子宫体积,改善患者血红蛋白水平。Objective To analyze the short-term and long-term efficacy of gonadotropin-releasing agent (GnRH-a) combined with levonorgestrel intrauterine system (LNG-IUS) in the treatment of adenomyosis. Methods A total of 60 patients with adenomyosis who were admitted to the hospital from December 2018 to November 2020 were randomly selected, and the patients were divided into observation group (n=30) and control group (n=30) by random number table method. The control group received LNG-IUS treatment, and the observation group was additionally treated with GnRH-a. The short-term and long-term efficacy, menstrual volume, dysmenorrhea score, uterine volume and hemoglobin changes were compared between the two groups. Results The total effective rate of patients in the observation group after 6 months and 12 months of treatment (93.33%, 86.21%) was higher than that in the control group, the difference was statistically significant (χ^(2)=3.456, 4.028, P<0.05). After 6 months and 12 months of treatment, the menstrual flow in the observation group was significantly lower than that in the control group, the difference was statistically significant (t=2.763, 3.426, P<0.05). After 6 months and 12 months of treatment, the degree of dysmenorrhea in the observation group was significantly lower than that in the control group, the difference was statistically significant (t=3.913, 3.790, P<0.05). After 6 months and 12 months of treatment, the uterine volume of the observation group was significantly lower than that of the control group, the difference was statistically significant (t=4.032, 9.940, P<0.05). After 6 months and 12 months of treatment, the hemoglobin levels in the observation group were significantly higher than those in the control group, the difference was statistically significant (t=6.490, 7.952, P<0.05). Conclusion GnRH-a combined with LNG-IUS in the treatment of patients with adenomyosis can significantly improve the short-term and long-term efficacy of patients with adenomyosis compared with LNG-IUS alone, maintain
