摘 要:目的 探讨柯萨奇B3病毒(CV-B3)感染与冠心病患者冠状动脉病变程度及炎性反应的关系。方法 本院2019年9月-2020年9月收治的50例冠心病患者作为观察组,同期医院体检的健康人员49人作为对照组。分别采用实时荧光定量PCR法和化学发光法检测两组患者CV-B3感染情况和血清炎性因子水平;采用冠状动脉造影对观察组冠状动脉病变程度进行Gensini评分,分析CV-B3感染与冠心病患者冠状动脉病变程度及炎性反应的关系。结果 观察组患者CV-B3感染率24.00%,对照组为8.16%,差异有统计学意义(P<0.05)。观察组中心肌梗死、心绞痛及隐匿性冠心病患者CV-B3感染率分别为50.00%、16.67%和8.33%,差异有统计学意义(P<0.05)。观察组患者感染后4、12、24 hHL-1心肌细胞及间充质干细胞的CV-B3基因拷贝数与对照组比较差异具有统计学意义(均P<0.05)。观察组CV-B3阳性患者的Gensini评分高于阴性患者(P<0.05)。观察组患者超敏C-反应蛋白(HS-CRP)及同型半胱氨酸(HCY)含量均高于对照组(均P<0.05)。观察组的TNF-α和IL-6水平均高于对照组(均P<0.05)。结论 冠心病患者CV-B3感染可加重冠状动脉病变程度,增强炎性反应,不利于患者康复。Objective To investigate the relationship between Coxsackie B3 virus(CV-B3) infection and the severity of coronary artery disease and inflammatory response in patients with coronary heart disease.Methods Fifty patients with coronary heart disease admitted to our hospital from September 2019 to September 2020 served as the observation group,and 49 healthy people who received physical examination in the hospital during the same period served as the control group.Real-time fluorescence quantitative PCR and chemiluminescence were used to detect CV-B3 infection and serum inflammatory factor levels in the two groups of patients;coronary angiography was used to perform Gensini scores on the degree of coronary artery disease in the observation group,to analyze the relationship between CV-B3 infection and either the degree of coronary artery disease or inflammatory response in patients.Results The CV-B3 infection rate was 24.00% in the observation group and 8.16% in the control group.The difference was statistically significant(P<0.05).The CV-B3 infection rates of patients with myocardial infarction,angina pectoris and occult coronary heart disease in the observation group were 50.00%,16.67% and 8.33%,respectively,and the difference was statistically significant(P<0.05).The CV-B3 gene copy number of HL-1 cardiomyocytes and mesenchymal stem cells in the observation group were significantly different from those in the control group at 4,12,and 24 h after infection(all P<0.05).The Gensini score of CV-B3 positive patients in the observation group was higher than that of negative patients(P<0.05).The contents of hypersensitive C-reactive protein(HS-CRP) and homocysteine ?(HCY) in the observation group were higher than those in the control group(both P<0.05).The levels of TNF-α and IL-6 in the observation group were higher than those in the control group(both P<0.05).Conclusion CV-B3 infection in patients with coronary heart disease can aggravate the severity of coronary artery disease and enhance the inflammatory response,whi
