摘 要:目的:研究3D打印导板辅助下空心螺钉内固定治疗复杂骨盆骨折患者的效果及其对骨盆功能的影响及安全性。方法:选取我科2019年1月至2020年12月期间收治的95例复杂骨盆骨折患者作为研究对象,按随机数字表法分为对照组(n=47)和观察组(n=48),对照组通过术前CT图像对骨盆骨折进行评估,并行内固定治疗,观察组在3D打印导板辅助下行空心螺钉内固定治疗,对比两组围手术指标、骨盆功能、并发症。结果:观察组的手术时间和住院时间短于对照组,出血量少于对照组,切口长度短于对照组(P<0.05);术后6个月,观察组的骨盆骨折复位Matta评分的优良率高于对照组,骨盆功能Majeed评分的优良率高于对照组(P<0.05);观察组的并发症发生率明显低于对照组(P<0.05)。结论:3D打印导板辅助下空心螺钉内固定治疗复杂骨盆骨折,可缩短手术时间,手术切口小,利于患者术后快速康复出院,且并发症少,患者的骨盆功能恢复较好。Objective:To study the effect of hollow screw internal fixation assisted by 3D printing guide plate in the treatment of patients with complex pelvic fractures and its impact on pelvic function and safety.Methods:95 patients with complex pelvic fractures treated in our department from January 2019 to December 2020 were selected as the research objects.They were randomly divided into control group(n=47)and observation group(n=48).The control group evaluated the pelvic fractures through preoperative CT images and treated with internal fixation.The observation group was treated with hollow screw internal fixation assisted by 3D printing guide plate,The perioperative indexes,pelvic function and complications were compared between the two groups.Results:The operation time and hospital stay in the observation group were shorter than those in the control group,the amount of bleeding was less than that in the control group,and the incision length was shorter than that in the control group(P<0.05);Six months after operation,the excellent and good rate of Matta score of pelvic fracture reduction in the observation group was higher than that in the control group,and the excellent and good rate of Majeed score of pelvic function in the observation group was higher than that in the control group(P<0.05);The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:3D printed guide plate assisted hollow screw internal fixation for the treatment of complex pelvic fractures can shorten the operation time,have a small surgical incision,and facilitate the rapid recovery and discharge of patients after surgery.There are fewer complications and the patient's pelvic function recovery is better.
