摘 要:目的探究尿液干化学及尿沉渣联合检测对尿道炎患者诊断准确率的影响。方法选取洛阳市孟津区人民医院166例疑似尿道炎患者(2019年1月至2021年12月)作为研究对象,均经尿液细菌培养明确病情,采集所有患者晨起空腹新鲜中段尿液标本,均行尿液干化学法检查、尿沉渣法检查。比较尿液干化学法检查、尿沉渣法检查尿常规指标(尿白细胞、尿红细胞),以尿液细菌培养为“金标准”,统计尿液干化学法、尿沉渣法单独检查及联合检查诊断尿道炎的检查结果、诊断效能。结果两种检查方式检查尿白细胞、尿红细胞水平比较,差异无统计学意义(P>0.05);166例疑似尿道炎患者经尿液细菌培养证实128例阳性、38例阴性,经尿液干化学检查检出117例阳性、49例阴性,经尿沉渣法检查检出110例阳性、56例阴性,经尿液干化学法+尿沉渣法联合检查检出132例阳性、34例阴性;尿液干化学法+尿沉渣法联合检查诊断尿道炎灵敏度98.44%、准确率95.18%高于尿液干化学法检查(88.28%、89.47%)、尿沉渣法检查(81.25%、84.21%),漏诊率1.56%低于尿液干化学法检查11.72%、尿沉渣法检查18.75%,差异有统计学意义(P<0.05)。结论尿道炎患者采用尿液干化学法+尿沉渣法联合检查可提高诊断灵敏度、准确率,降低漏诊率,可为临床诊断提供有效信息支持,有利于指导临床治疗,改善患者预后。Objective To investigate the effect of combined urine dry chemistry and urine sediment detection on the diagnostic accuracy of urethritis.Methods A total of 166 suspected urethritis patients from Mengjin District People's Hospital of Luoyang City(January 2019 to December 2021)were selected as the study subjects.The condition of all patients was determined by urine bacterial culture.Fasting fresh mid-stream urine samples were collected from all patients in the morning,and urine dry chemical test and urine sediment test were performed.The results and diagnostic efficiency of urine dry chemistry method and urine sediment method in diagnosing urethritis were compared,and urine bacterial culture was taken as the"gold standard".Results Among 166 suspected urethritis patients,128 were positive and 38 were negative by urine bacterial culture,117 were positive and 49 were negative by urine dry chemical examination,110 were positive and 56 were negative by urine sediment examination,132 were positive and 34 were negative by urine dry chemical and urine sediment combined examination.The sensitivity and accuracy of urine dry chemistry combined with urine sediment were 98.44%and 95.18%higher than those of urine dry chemistry(88.28%,89.47%)and urine sediment(81.25%,84.21%).The rate of missed diagnosis(1.56%)was significantly lower than that of urine dry chemical method(11.72%)and urine sediment method(18.75%)(P<0.05).Conclusion The combined urine dry chemistry and urine sediment method can improve the diagnostic sensitivity and accuracy,reduce the rate of missed diagnosis,provide effective information support for clinical diagnosis,guide clinical treatment and improve the prognosis of patients with urethritis.
