摘  要:胆道外科的发展趋势是微创化,以最小创伤来解决复杂的临床问题。术中超声在胆道外科的应用大大推进了胆道外科微创化的发展,形成了多种依赖于术中超声的新型术式。术中超声可在术中对胆管结石、肿瘤、炎症、胆管变异等关键结构精准定位并引导相关操作(如引导胆道镜及器械取石、置管引流、穿刺活检、肿瘤消融)。术中超声可指导选择最佳断肝平面及脉管结扎点,最大限度地保留重要脉管结构及残肝功能。随着精准胆道外科的发展,术中超声将会向探头微小化、术中穿刺技术、超声造影、弹性成像、融合导航等方向发展,使手术更加微创、精准和高效。The development trend of biliary surgery is minimally invasive,to solve complex clinical problems with minimal trauma.The application of intraoperative ultrasound in biliary surgery has greatly promoted the development of minimally invasive biliary surgery and formed a variety of new surgical methods dependent on intraoperative ultrasound.Intraoperative ultrasound can accurately locate the key structures of bile duct stones,tumors,inflammation,bile duct variation during surgery and guide relevant operations(such as guiding choledochoscope and instruments for stone removal,catheterization and drainage,puncture biopsy,and tumor ablation).Intraoperative ultrasound can guide the selection of optimal hepatic fracture plane and vascular ligation site to maximize the retention of important vascular structure and residual liver function.With the development of precision biliary surgery,intraoperative ultrasound will develop in the direction of probe miniaturization,intraoperative puncture technology,contrast-enhanced ultrasound,elastography,fusion navigation,etc.,so as to make the surgery more minimally invasive,accurate and efficient.
