TESSYS技术对腰椎间盘突出合并神经根管狭窄症患者的下肢神经功能及腰椎功能的影响

摘　　要：目的 探究经椎间孔内窥镜脊柱系统(TESSYS)技术对腰椎间盘突出(LDH)合并神经根管狭窄症(LDHLRS)患者的下肢神经功能及腰椎功能的影响。方法 选取我院2017年7月至2019年7月126例LDH合并LDHLRS患者为研究对象,依据手术方法不同分为常规组和干预组。常规组行经椎间孔腰椎椎体间融合术(TLIF)治疗,干预组行TESSYS技术治疗,对比两组下肢神经功能、腰椎功能,手术并发症。结果 两组术后1年末腓总神经、胫神经的MCV及隐神经、腓肠神经的SCV水平较术前升高,干预组高于常规组(P<0.05)。两组术后1年末ODI、腰痛、腿痛VAS评分水平较术前降低,干预组低于常规组;JOA评分水平较术前升高,干预组高于常规组(P<0.05)。干预组并发症率1.59%低于常规组6.35%,差异无统计学意义(P>0.05)。结论 TESSYS技术通过解压神经根管以改善LDH合并LDHLRS患者的腰椎功能,改善下肢神经传导速度,降低术后并发症的发生,提高临床疗效。Objective To explore the effect of transforaminal endoscopic spinal system(tessys)technology on lower limb nerve function and lumbar function in patients with lumbar disc herniation(LDH)combined with nerve root canal stenosis(ldhlrs).Methods 126 patients with LDH and ldhlrs in our hospital from July 2017 to July 2019 were selected as the research objects and divided into routine group and intervention group according to different surgical methods.The conventional group was treated with transforaminal lumbar interbody fusion(TLIF),and the intervention group was treated with tessys technology.The lower limb nerve function, lumbar function and surgical complications of the two groups were compared.Results The levels of MCV of common peroneal nerve and tibial nerve, SCV of saphenous nerve and sural nerve in the two groups at the end of 1 year after operation were higher than those before operation, and the intervention group was higher than the conventional group(P<0.05).At the end of one year after operation, the ODI,VAS scores of low back pain and leg pain in the two groups were lower than those before operation, and the JOA scores in the intervention group were higher than those in the conventional group(P<0.05).The complication rate of the intervention group was 1.59%,which was lower than that of the conventional group(6.35%),and the difference was not statistically significant(P>0.05).Conclusion Tessys technique can improve the lumbar function of LDH patients with ldhlrs by decompressing the nerve root canal, improve the nerve conduction velocity of lower limbs, reduce the incidence of postoperative complications and improve the clinical efficacy.
