原发性高血压患者中性粒细胞/淋巴细胞、血小板/淋巴细胞、平均血小板体积与左心室肥厚的相关性研究

摘　　要：目的 探讨中性粒细胞/淋巴细胞(NLR)、血小板/淋巴细胞比值(PLR)和平均血小板体积(MPV)与原发性高血压患者左心室肥厚(LVH)之间的关系。方法 选取400例原发性高血压患者,对入选者行心脏超声检查,根据左心室质量指数(LVMI)分为左心室肥厚组(196例)和左心室正常组(204例),检测所有入选者的血常规,计算NLR和PLR。结果 在高血压患者中,左心室肥厚组NLR、PLR和MPV均高于左心室正常组(P<0.05);Pearson相关分析提示,LVMI与NLR(r=0.336,P<0.001)、PLR(r=0.323,P<0.001)和MPV(r=0.297,P=0.002)呈正相关;Logistic回归分析提示NLR (OR=2.995,95%CI 2.218~4.643,P<0.001)、PLR (OR=2.356,95%CI 1.968~3.571,P=0.006)和MPV (OR=2.017,95%CI 1.528~2.533,P=0.011)是高血压者合并左心室肥厚的独立危险因素;ROC曲线分析提示NLR对原发性高血压患者发生左心室肥厚的预测价值优于PLR和MPV。结论 在原发性高血压患者中,NLR、PLR和MPV与左心室肥厚密切相关。Objective To explore the association of left ventricular hypertrophy(LVH) with neutrophil to lymphocyte ratio(NLR),platelet-to-lymphocyteratio(PLR) and meanplateletvolume(MPV)in patients with hypertension.Methods Totally 400 patients with hypertension were divided into two groups according to left ventricular myocardial mass index(LVMI):LVH group(n=196) and non-LVH group(n=204).Blood routine tests were performed for all participants and then NLR and PLR were calculated.Results NLR,PLR and meanplateletvolumewere significantly higher in the LVH group than the non-LVH group(P<0.05).Pearson correlation analysis showed that LVMI was positively correlated with NLR(r=0.336,P<0.001),PLR(r=0.323,P<0.001) and MPV(r=0.297,P=0.002).Logistic analysis showed that NLR(OR= 2.995,95%CI 2.218-4.643,P<0.001),PLR(OR=2.356,95%CI 1.968-3.571,P=0.006) and MPV(OR=2.017,95%CI 1.528-2.533,P=0.011) were independent risk factors for LVH in hypertensive patients after adjusted for confounding factors.Conclusion NLR,PLR and MPV are significantly correlated with LVH in patients with hypertension.
