摘　　要：目的分析儿童遗传性难治性溶血性贫血患者输注红细胞效果的影响因素,为临床治疗提供参考依据。方法回顾性研究2015年1月~2021年5月,在本院输注红细胞的儿童遗传性溶血性贫血患者的病历资料,共65例。根据红细胞输注效果,分为有效组和无效组,对可能影响输注效果的因素(性别、年龄、血型、输注前血红蛋白、血液成分、输注次数、溶贫类型、脾大)行单因素和多因素Logistic回归分析。结果65例患者输注80例次,单因素分析结果显示脾大对于输注效果的影响差异有统计学意义(P<0.05)。多因素Logistic回归分析显示脾大为输注效果的独立危险因素(OR=14.759,95%CI:3.527~61.752,P<0.001)。结论脾大是儿童遗传性难治性溶血性贫血的红细胞输注效果的影响因素,对临床输血及该疾病的治疗有参考意义。Objective To analyze factors influencing the effects of blood transfusions on hereditary refractory hemolytic anemia in children,so as to provide experimental data.Methods We retrospectively analysed the medical files for 65 children with hereditary hemolytic anemia(HHA)in our hospital from January 2015 to May 2021.Patients were divided into effective group and ineffective group based on the efficacy of blood transfusion.Univariate and multivariate logistic regression analyses were performed to evaluate the factors(including gender,age,blood type,hemoglobin before transfusion,blood composition,times,type of hemolytic anemia,splenomegaly)associated with transfusion efficacy.Results Sixty-five patients had performed blood transfusion 80 times.The univariate analysis showed that splenomegaly had a significant impact(P<0.05).The multivariate analysis showed that splenomegaly(OR=14.759,95%CI:3.527~61.752,P<0.001)was an independent risk factor for transfusion efficacy.Conclusion Splenomegaly is a factor influencing blood transfusion in HHA children,which has reference significance for transfusion and treatment of disease.
