摘 要:目的探讨以骨髓干细胞辅以生物活性因子组成的促血管生成疗法对大鼠缺血后肢运动功能的影响。方法24只雌性SD大鼠切除右后肢股动脉及其分支制作后肢缺血模型,每3 d记一次双下肢足背与足底的皮温,成模后第60天随机数表法分为3组(每组8只),给予缺血的后肢内外两侧肌肉注射干预,NS组注射生理盐水空白对照,BSCs组注射纯骨髓干细胞混悬液,HGI-BSCs组注射加有透明质酸(HA)、单核细胞集落刺激因子(GM-CSF)、白细胞介素(IL)-2因子的骨髓干细胞混悬液。干预治疗后第60天,对3组大鼠进行"一次性运动力竭"试验,记录各组大鼠跑步距离。过麻处死大鼠取缺血后肢肌肉观察微血管密度(MVD),组间比较采用方差分析和SNK检验。结果干预治疗后,NS组健侧后肢皮温[足背:(28.85±0.38)℃,足底:(28.91±0.41)℃]高于缺血后肢体皮温[足背:(26.70±0.36)℃,足底:(27.15±0.33)℃],差异有统计学意义(t=4.063、3.317,P<0.05);BSCs组健侧后肢[足背:(27.79±0.30)℃,足底:(27.84±0.30)℃]与缺血后肢体皮温[足背:(27.26±0.36)℃,足底:(27.54±0.38)℃],差异无统计学意义(t=1.116、0.618,P>0.05);HGI-BSCs组健侧后肢[足背:(29.34±0.65)℃,足底:(29.48±0.65)℃]与缺血后肢体皮温[足背:(28.80±0.59)℃,足底:(29.11±0.66)℃],差异无统计学意义(t=0.611、0.390,P>0.05)。在运动功能方面:HGI-BSCs组[(1540.00±90.51)m]高于BSCs组[(753.10±60.42)m],差异有统计学意义(t=10.540,P<0.01);BSCs组[(753.10±60.42)m]高于NS组[(233.00±84.81)m],差异有统计学意义(t=4.995,P<0.01)。MVD计数:HGI-BSCs组(25.33±0.52)高于BSCs组(17.53±0.43),差异有统计学意义(t=23.520,P<0.01);BSCs组(17.53±0.43)高于NS组(8.80±0.47),差异有统计学意义(t=13.640,P<0.01)。结论促血管生成疗法可改善大鼠缺血后肢皮温、促进血管生成和改善其运动功能。Objective To investigate the effects of an angiogenic therapy consisting of bone marrow stem cells(BSCs)supplemented with bioactive factor on motor function of ischemic hindlimb in rats.Methods The femoral arteries and their branches of the right hind limb were excised in 24 female SD rats to make the model of hindlimb ischemia.The skin temperature of the dorsus and plantar of both lower limbs were recorded every 3 days.On the 60th day after modeling,the animals were randomly divided into 3 groups(8 rats in each group),and intramuscular injection was given to both sides of the ischemic hind limb.The blank control group was injected with normal saline,the BSCs group was injected with only BSCs suspension,and the HGI-BSCs group was injected with the BSCs suspension supplemented with HA,GM-CSF,and IL-2.On the 60th day after the intervention,the three groups of rats were tested with a disposable exercise exhaustive test.The running distance of each group of rats was recorded.After the rats were sacrificed by anesthesia,the ischemic hindlimb muscles were taken to observe the microvascular density(MVD).Results After treatment,the skin temperature of the healthy hind limb[dorsally:(28.85±0.38)℃,plantar:(28.91±0.41)℃]in the blank control group was significantly higher than that in the ischemic limb[dorsally:(26.70±0.36)℃,plantar:(28.91±0.41)℃,t=4.063,3.317,P<0.05].There was no statistically significant difference in the terminal skin temperature between the ischemic limb[dorsally:(27.79±0.30)℃,plantar:(27.84±0.30)℃]and the normal limb[dorsally:(27.26±0.36),(27.54±0.38)℃]in the BSCs group(t=1.116,0.618,P>0.05)and the HGI-BSCs group[dorsally:(29.34±0.65)℃vs.(28.80±0.59)℃,plantar:(29.48±0.65)℃vs.(29.11±0.66)℃,t=0.611,0.390,P>0.05].In terms of motor function,the running distance of HGI-BSCs group[(1540.00±90.51)m]was significantly longer than that of BSCs group[(753.10±60.42)m,t=10.54,P<0.01],and that of BSCs group[(753.10±60.42)m]was significantly longer than that of blank control group[
