摘 要:目的探讨高频彩色多普勒超声对类风湿关节炎(RA)膝关节病变的诊断价值。方法选取在我院就诊的86例疑似RA膝关节病变患者作为研究对象,依据诊断金标准将其分为阳性组(RA,43例,86个关节)和阴性组(非RA,43例,86个关节),两组均进行高频彩色多普勒超声检查。比较阳性组和阴性组的滑膜情况;比较阳性组不同活动障碍患者的滑膜厚度、关节腔积液、骨质破坏情况及滑膜内血流信号。结果阳性组的滑膜厚度、滑膜病变率均高于阴性组,同时滑膜厚度Ⅰ、Ⅱ、Ⅲ级比例均高于阴性组(P<0.05)。重度活动障碍组的滑膜厚度、关节腔积液率、骨质破坏率、滑膜内血流信号异常率均高于中/轻度活动障碍组(P<0.05)。高频彩色多普勒超声诊断RA膝关节病变的灵敏度、特异度分别为95.35%、96.67%。结论高频彩色多普勒超声可准确、灵敏地检测出膝关节滑膜病变、关节腔积液及骨质破坏情况,清晰显示出滑膜内血流信号,可为RA膝关节病变诊断提供科学依据。Objective To explore the diagnostic value of high frequency color Doppler ultrasound in rheumatoid arthritis(RA)knee joint lesions.Methods A total of 86 patients with suspected RA knee joint lesions who visited in our hospital were selected as the research objects.The patients were divided into positive group(RA,43 cases,86 joints)and negative group(non-RA,43 cases,86 joints)according to the diagnostic gold standard,and high frequency color Doppler ultrasound were performed in both groups.The synovial conditions of the positive group and the negative group were compared;the synovial thickness,joint effusion,bone destruction and blood flow signal in the synovium were compared in the positive group with different movement disorders.Results The synovial thickness and synovial lesions rate in the positive group were higher than those in the negative group,and the proportions of gradesⅠ,ⅡandⅢsynovial thickness were higher than those in the negative group(P<0.05).The synovial thickness,joint effusion rate,bone destruction rate and abnormal blood flow signal rate in the synovium in the severe mobility impairment group were higher than those in the moderate/mild mobility impairment group(P<0.05).The sensitivity and specificity of high frequency color Doppler ultrasound in the diagnosis of RA knee join lesions were 95.35%and 96.67%.Conclusion High frequency color Doppler ultrasound can accurately and sensitively detect knee joint synovial lesions,joint effusion and bone destruction,and clearly display the blood flow signal in the synovium,which can provide a scientific basis for the diagnosis of RA knee joint lesions.
