摘 要:目的观察踏车运动联合呼吸训练在老年维持性血液透析(MHD)患者中的应用效果。方法将在本院血透中心接受MHD治疗的57例老年患者采用随机数字表法分为对照组(28例)和观察组(29例)。对照组给予踏车运动,观察组在对照组基础上增加呼吸训练。比较两组的肺功能指标,包括第1秒用力呼气容积(FEV1)、最大呼气流速峰值(PEF)、用力肺活量(FVC),所有结果以测得的实际值占预计值的百分比表示,即FEV1%、PEF%、FVC%;比较两组的运动耐量、疲乏程度、生活质量。结果干预后,观察组的FEV1%、PEF%、FVC%高于对照组(P<0.05)。干预后,两组的峰值摄氧量(VO2peak)、无氧阈升高,且观察组高于对照组(P<0.05)。干预后,观察组的疲乏量表-14(FS-14)评分低于干预前和对照组,肾脏疾病生活质量评估简表(KDQOL-SF)评分高于干预前和对照组(P<0.05)。结论在踏车运动基础上增加呼吸训练能有效促进老年MHD患者肺功能指标、运动耐量、疲乏程度和生活质量改善。Objective To observe the application effect of treadmill exercise combined with breathing training in elderly patients with maintenance hemodialysis(MHD).Methods A total of 57 elderly patients who received MHD treatment in hemodialysis center of our hospital were divided into control group(28 cases)and observation group(29 cases)by random number table method.The control group was given treadmill exercise,and the observation group was added with breathing training on the basis of the control group.Lung function indexes of the two groups were compared,including forced expiratory volume in the first second(FEV1),peak expiratory flow(PEF)and forced vital capacity(FVC),all results were expressed as the percentage of the measured actual value in the predicted value,namely FEV1%,PEF%and FVC%;the exercise tolerance,fatigue degree and quality of life were compared between the two groups.Results After intervention,FEV1%,PEF%and FVC%in the observation group were higher than those in the control group(P<0.05).After intervention,peak oxygen uptake(VO2peak)and anaerobic threshold of the two groups increased,and those of the observation group were higher than the control group(P<0.05).After intervention,the Fatigue Scale-14(FS-14)score of the observation group was lower than that before intervention and the control group,and the Kidney Disease Quality of Life Short Form(KDQOL-SF)score was higher than that before intervention and the control group(P<0.05).Conclusion Adding breathing training on the basis of treadmill exercise can effectively improve lung function indexes,exercise tolerance,fatigue degree and quality of life in elderly MHD patients.
