摘　　要：目的 归纳粘连性中耳炎(adhesive otitis media,AdOM)患者的临床特征,总结鼓室成形术对AdOM患者的治疗效果,为制定AdOM的外科处理方案提供依据。方法 回顾分析2016年1月~2018年12月于北京大学第三医院诊断为AdOM并接受鼓室成形术的患者15例(15耳),总结患者的临床资料,分析其术后干耳情况、听力变化、并发症发生情况等。结果 15例患者随访时间13~49个月,至随访结束时所有患者均获得干耳,鼓膜均愈合良好。术前气导听阈为(50.1±20.5)dB,术后降低至(41.8±22.7)dB。术前气骨导差(air-bone gap,ABG)为(31.6±11.6)dB,术后ABG为(23.8±11.5)dB,其中有5耳达到ABG闭合。所有患者术后未发生手术并发症。结论 严格掌握适应证,术前精准的评估,鼓室成形术对于部分AdOM患者具有较好的治疗效果。OBJECTIVE To summarize the clinical characteristics of and therapeutic effect of tympanoplasty on adhesive otitis media(AdOM),so as to provide evidence for the surgical management of AdOM.METHODS Fifteen patients(15 ears) diagnosed with AdOM and underwent tympanoplasty in our hospital from January 2016 to December 2018 were retrospectively analyzed.The clinical data of the patients were summarized,and otorrhea condition,hearing changes and complications after surgery were analyzed.RESULTS The 15 patients were followed up for 13 to 49 months.All the patients achieved a dry ear at the last visit.The tympanic membrane healed well.The threshold of air conduction(PTA) was(50.1±20.5)dB before operation and decreased to(41.8±22.7)dB after operation.The pre-operative and post-operative air-bone gap(ABG)was(31.6±11.6)dB and(23.8±11.5)dB respectively.ABG closure was achieved in 5 ears.No surgical complications occurred in all patients.CONCLUSION By strictly controlling the surgical indications and accurate preoperative evaluation,tympanoplasty has a good therapeutic effect for some patients with AdOM.
