摘 要:目的评价潮气肺功能数据对哮喘患儿的早期诊断价值.方法纳入2021年1月-2021年10月在佛山市妇幼保健院呼吸科治疗的32例哮喘患儿为研究对象(哮喘组),选取年龄相似的28例肺炎患儿为对照组.根据疾病的发展阶段不同,将哮喘组患儿分为疾病急性发作期亚组(n=20)与疾病缓解期亚组(n=12).比较哮喘组与对照组、急性发作期亚组与缓解期亚组潮气肺功能指标,包括呼气峰流速时呼出的容积(VPTEF)与呼气容积(VE)比值、呼气峰流速的时间(TPTEF)与呼气时间(TE)的比值及每千克体质量潮气量(VT).采用ROC曲线评价以上潮气肺功能指标单独诊断、联合诊断早期哮喘的价值,评价以上潮气肺功能指标单独诊断、联合诊断哮喘急性期与哮喘缓解期的价值.结果哮喘组VPTEF/VE(t=2.249,P=0.028)、TPTEF/TE(t=2.240,P=0.029)与VT(t=2.269,P=0.027)较对照组显著降低.急性发作亚组VPTEF/VE(t=2.223,P=0.034)、TPTEF/TE(t=2.418,P=0.022)与VT(t=2.612,P=0.014)较缓解期亚组显著降低.潮气肺功能指标联合诊断早期哮喘的准确性最高(AUC=0.796,95%CI:0.146-0.289,敏感度为70.8%,特异度为76.1%).潮气肺功能指标联合诊断哮喘缓解期和哮喘急性期的准确性最高(AUC=0.860,95%CI:0.422-0.928,敏感度为80.6%,特异度为87.4%).结论潮气肺功能指标诊断早期哮喘患儿具有较高的准确性,同时也能够准确判断哮喘的严重程度和哮喘分期.Objective To evaluate the value of tidal lung function data in the early diagnosis of children with asthma.Methods A total of 32 children with asthma who were treatec]in the respiratory ciepartment of our hospifal from January 2021 to October 2021 were included as the research object(asthma group),and 28 children with pneumonia with similar age were selected as the control group.According to the different stages of disease development,children in the asthma group were div ided into acute onset subgroups(n=20)and remission subgroups(n=12).Compare the tidal lung function indexes of the asthma group and the control group,the acute atack subgroup and the remission subgroup.including the ratio of expiratory volume at peak expiralory flow rate(VPTEF)to expiratory volume(TE),and the time of peak expirtory flow rate(TPTEF)and expiralory time(TE)raio and tidal volume per kilogram of body mass(VT).The ROC curve was used to evaluate the value of the above tidal lung function indicators in the single diagnosis and joint diagnosis of early asthma.and the value of the ahove tidal lung funetion indicators in the single diagnosis and joint diagnosis of asthma in the acute phase and asthma remission phase.Results VPTEFNE(=2.249.P=0.028),TPTEF/TE(1=2.240,P=0.029)and VT(=2.269,P=0.027)in the asthma group were signifcantly lower than those in the control group.The acute onset subgroup VPTEF/VE(1=2.223.P=0.034),TPTEF/TE(=2.418.,P=0.022)and VT(1=2.612P=0.014)were signifcanly lower than those in the remission subgoup.The combinaion of tidal lung function indexes has the highest aceuracy in diagnosing early asthma(AUC=0.796,95%CI:0.146-0.289,sensitivity is 70.8%speifeity is 76.1%).The combination of tidal lung function indexes in the diagnosis of asthma in remission and acute asthma has the highest accuracy(AUC=0.860.95%CL:0.422-0.928.sensitivity 80.6%speifeity 87.4%).Conclusion Tidal lung funetion indicators have high accuracy in diagnosing ehildren with early ashma,and they can also acurately judge the severily of ashma and the stage
