托里消毒散加减对慢性化脓性中耳炎(脾虚湿困型)术后干耳的作用

摘　　要：目的:观察托里消毒散加减对脾虚湿困型慢性化脓性中耳炎术后干耳的作用。方法:选择慢性化脓性中耳炎(脾虚湿困型)患者61例(61耳),其中试验组30例(30耳),对照组31例(31耳),两组均行改良乳突根治术加鼓室成形术,术后两组患者均行抗感染、耳内镜下换药等治疗,试验组则在此基础上,口服托里消毒散加减中药汤剂8周。观察两组患者术后干耳时间,术后第2、4、8、12周各时间点的干耳率,局部和全身症状的VAS评分情况。结果:与对照组相比,试验组能够缩短干耳时间(P<0.05),提高术后第8周时的干耳率(P<0.05),改善术后第8周耳渗出(或流脓)VAS评分(P<0.05),术后第4、8、12周自觉耳闷VAS评分(P<0.05),以及术后第2、4、8、12周全身症状VAS评分(P<0.05)。结论:托里消毒散加减可以缩短脾虚湿困型慢性化脓性中耳炎患者术后干耳时间,提高术后早期干耳率,并可有效改善术后症状,值得临床借鉴。Objective:To observe the effect of modified Tuoli Xiaodu powder on dry ear after operation of chronic suppurative otitis media with spleen deficiency and dampness stagnation.Methods:61cases(61ears)of chronic suppurative otitis media(spleen deficiency and dampness trap type)were selected,including 30cases(30ears)in the experimental group and 31cases(31ears)in the control group.Both groups were treated with modified radical mastoidectomy and tympanoplasty.After operation,the patients in both groups were treated with anti infection and dressing change under otoscope.On this basis,the experimental group took Tori Xiaodu powder plus or minus traditional Chinese medicine decoction for 8 weeks.The postoperative dry ear time,dry ear rate at 2,4,8and 12weeks after operation,and VAS scores of local and systemic symptoms were observed.Results:Compared with the control group,the experimental group could shorten the dry ear time(P<0.05),improve the dry ear rate at the 8th week after operation(P<0.05),improve the VAS score of ear exudation(or pus)at the 8th week after operation(P<0.05),the VAS score of conscious ear tightness at the 4th,8th and 12th weeks after operation(P<0.05),and the VAS score of systemic symptoms at the 2nd,4th,8th and 12th weeks after operation(P<0.05).Conclusion:The addition and subtraction of Tuoli disinfection powder can shorten the postoperative dry ear time of patients with chronic suppurative otitis media of spleen deficiency and dampness stagnation type,improve the early dry ear rate,and effectively improve the postoperative symptoms,which is worthy of clinical reference.
