摘  要:目的探索压缩感知技术(CS)高倍数加速因子(AF)对3D NerveVIEW臂丛神经MRI图像质量及扫描时间的影响。方法招募受检者24例[男10例,平均年龄(45.92±16.73)岁],其中17例健康受检者,7例臂丛损伤受检者。3.0 T MR行结合平行采样技术(SENSE)以及CS的臂丛神经3D NerveVIEW序列扫描,采用的AF分别为SENSE=2,CS=2、4、6、8、10、12。两观察者在原始图上划定感兴趣区(ROI)测量信号强度(SI)和标准差(SD),并计算信噪比(SNR)和对比噪声比(CNR)。并对图像质量进行四分法主观评分。组内相关系数(ICC)、Kappa检验两位观察者测量数据和主观评分的一致性,若一致性良好,选择高年资医师主观评分进行后续分析。使用Friedman检验不同AF之间SNR、CNR及主观评分的差异性。若差异性有统计学意义,使用最小差异性检验(LSD法)进行两两比较。结果两位观察者测量数据和主观评分一致性良好(ICC 0787~0.995,Kappa 0.778~0.882)。不同AF之间SNR、CNR和主观评分差异有统计学意义(P<0.001)。当CS=8时,左右侧神经干SNR较常规SENSE=2差异有统计学意义(P<0.05);当CS=10时,左右侧神经干CNR,左右侧神经根SNR、CNR及主观评分较SENSE=2差异有统计学意义(P<0.05)。结论结合CS的臂丛神经3D NerveVIEW序列随着AF的增加,扫描时间逐渐降低。CS=6对比SENSE=2,在确保图像质量前提下,扫描时间缩短64.31%,临床推荐使用CS=6行臂丛神经3D NerveVIEW序列扫描。Objective Exploring the Impact of Compressed SENSE(CS)on the Image Quality and scan time of Brachial Plexus 3 D Nerve VIEW Sequences.Methods A total of 24 subjects(10 males,mean age:45.92±16.73)were scanned using a 3.0 T MR scanner,including 17 healthy subjects and 7 subjects with brachial plexus injury.Each subject underwent a series of 3 D NerveVIEW scan with varying acceleration factors:SENSE=2,CS=2,4,6,8,10,12.Two observers delineated the ROI on the original image to measure the signal intensity(SI)and Standard Deviation(SD).Calculate the signal to noise ratio(SNR)and contrast to noise ratio(CNR).Subjectively score the image quality by quarter method.The intraclass correlation coefficient and Kappa test were adopted to evaluate the consistency of the scores from the two radiologists.If the consistency was in good agreement,the corresponding images would be adopted for further analysis by senior physicians.In the following analysis,the Friedman test was used to assess the difference of SNR,CNR and score between groups,and the LSD test was employed to make a pairwise comparison.Results The measurement data and scores of the two observers were in good agreement(ICC:0787-0.995,Kappa:0.778-0.882),it was shown that there were statistically significant differences in SNR,CNR and scores of different AF.When CS=8,the SNR of left and right nerve trunk were significantly different from those of conventional sequences(P<0.05);When CS=10,the SNR of left and right nerve trunk,SNR,CNR and supervisor scores of left and right nerve root had statistically significant differences compared with conventional sequences(P<0.05).Conclusion Scan time for the brachial plexus decreases gradually with the increase of the CS acceleration factor.CS factor of 6 was recommended for clinic to achieve an optimal balance between scan time and image quality,with scan time decreasing to 64.31%.
