脊髓栓系综合征患儿健康宣教效果的影响因素分析

摘　　要：目的分析脊髓栓系综合征患儿健康宣教效果的影响因素,减少因手术产生的患儿预后偏移,进一步提高治疗效果。方法采用整体随机抽样法选取2019年12月—2020年12月温州医科大学附属第二医院收治的不同分型脊髓栓系综合征且接受脊髓栓系松解术的95例患儿,所有患儿及陪伴监护人均给予术前和术后健康宣教,观察健康宣教效果,采用多因素非条件Logistic逐步回归模型分析脊髓栓系综合征患儿健康宣教效果的影响因素。结果健康宣教满意率为84.21%(80/95),不满意率为15.79%(15/95),差异有统计学意义(Z=6.529,P=0.023)。多因素Logistic回归分析结果显示:患儿年龄、陪伴监护人文化程度、陪伴监护类型、希望水平、心理弹性、家庭功能及健康宣教途径是健康宣教效果的影响因素(均P<0.05)。结论健康宣教在不同分型脊髓栓系综合征患儿手术中的应用效果满意,健康宣教效果的影响因素较多,根据影响因素,针对重点群体采取相应对策有利于提高宣教效果,减少因手术等原因产生的患儿预后偏移,提高治疗预期,促进患儿早日康复。Objective To analyze the influencing factors of health education effect in children with tethered cord syndrome,reduce prognostic deviation induced by surgery,further improve curative effect.Methods A total of 95 children with different types of tethered cord syndrome who underwent tethered cord release(TVP)in the Second Affiliated Hospital of Wenzhou Medical University from December 2019 to December 2020 were selected by cluster random sampling method,all the patients and accompanying guardians were given health education before and after operation,health education effect was observed,multivariate non-conditional Logistic stepwise regression model was used to analyze the influencing factors of health education effect in children with tethered cord syndrome.Results The satisfaction rate of health education was 84.21%(80/95),the dissatisfaction rate was 15.79%(15/95),there was statistically significant difference(Z=6.529,P=0.023).Multivariate Logistic regression analysis showed that age,educational levels of accompanying guardians,types of accompanying guardians,hope level,mental flexibility,family function,and ways of health propaganda and education were influencing factors of health education effect in children with tethered cord syndrome(P<0.05).Conclusion The application effect of health education in the operation of children with different types of tethered cord syndrome is satisfactory,the related factors affecting health education are various,and according to the influencing factors,corresponding countermeasures should be taken for the key groups,it can improve the effect of propaganda and education,reduce deviation of prognosis,improve treatment expectation,and promote early recovery of the patients.
