高频彩超在儿童一过性肠套叠诊断中的应用价值

摘　　要：目的:探讨高频彩超在儿童一过性肠套叠疾病的诊断中的作用和价值。方法:收集2017年9月—2021年2月期间在秀洲区王店、王江泾、新塍医院265例患儿303个一过性肠套叠和非一过性肠套叠的临床资料。结果:儿童一过性肠套叠性疾病的早期临床表现症状不典型,阵发性哭吵、腹痛、呕吐为多数患儿首发的临床症状;多数患儿的发病平均年龄为(3.7±2.3)岁,男患儿多于女患儿(1.18:1);秋冬季发病多见于春夏季(60%);肠套叠具有特异的超声波影像和血流信号表现,其中一过性肠套叠的“同心圆”平均直径(1.63±0.45) cm,套叠平均长度(2.77±0.95) cm;59例患儿经过探头推挤按压、顺时针手法按揉腹部、走动等方式,自行复位;跟踪随访1周,发现其中3例患者出现病情反复,再次复位后临床症状消失。结论:儿童一过性肠套叠在日常工作中并不少见,其可以自行脱出,不需要特别治疗;高频彩超在儿童一过性肠套叠疾病的诊断中具有重要的临床意义。Objective To explore the value of high-frequency color ultrasound in the diagnosis of transient intussusception disease in children.Methods Clinical data on 303 primary and intussusception were colected in 265 children in Wangdian Wang Jiangjing and Xincheng Hospital in Xiuzhou District from September 2017 to February 2021.Results The early clinical manifestations of childhood transient intussusception disease were not typical,paroxysmal crying,abdominal pain and vomiting were the first clinical symptoms of most children.The average age of onset in most children was(3.7±2.3) years old.It was more male children than female children(1.18:1).Autumn and winter diseases were often seen in spring and summer(60%).Intussusception had special ultrasound images and blood flow signals.The mean diameter of the overlapping "concentric circles" was(1.63±0.15) cm.Average casing length was(2.77±0.95) cm.59 cases of children were through the probe push pressing,clockwise massage abdomen,walking and other ways,self-reduction,follow-up for 1 week.Three of these patients were found to be repeated.Clinical symptoms disappeared after being reset again.Conclusion It is not uncommon for children in daily work,which can be removed without special treatment,highfrequency color ultrasound has great clinical significance in the diagnosis of the disease in children.
