摘　　要：目的:探讨来曲唑片联合益母草颗粒对多囊卵巢综合征(PCOS)患者的治疗效果及对外周血叉头转录因子O1(FOXO1)、血清淀粉样蛋白P(SAP)和血清死亡蛋白激酶1(DAPK1)表达的影响。方法:选取2017年8月至2020年3月中国人民武装警察部队特色医学中心收治的PCOS患者154例作为研究对象,按随机数字表法分为对照组(77例)和治疗组(77例)。对照组患者于月经来潮的第5日开始口服来曲唑片,1次5 mg, 1日1次,连服5 d,此为1个疗程;治疗组患者在对照组的基础上同时口服益母草颗粒,1次15 g, 1日2次,从月经来潮第5日起连续服用至下次月经来潮,此为1个疗程;两组患者均连续治疗3个疗程。比较两组患者治疗前后的临床疗效、性激素和糖代谢水平,以及FOXO1、SAP和DAPK1的表达情况。结果:治疗组患者的总有效率为98.70%(76/77),高于对照组(90.91%,70/77),差异有统计学意义(P<0.05)。与治疗前相比,两组患者治疗后性激素(血清黄体生成素、卵泡刺激素和睾酮)水平均降低,且治疗组患者治疗后性激素水平低于对照组,差异均有统计学意义(P<0.05)。与治疗前相比,两组患者治疗后空腹胰岛素(FINS)、胰岛素样生长因子1(IGF-1)水平及胰岛素抵抗指数(HOMA-IR)均降低,且治疗组患者治疗后的FINS、IGF-1水平及HOMA-IR低于对照组,差异均有统计学意义(P<0.05)。两组患者治疗后的血清FOXO1、SAP和DAPK1表达水平低于治疗前,且治疗组患者治疗后的FOXO1、SAP、DAPK1表达水平低于对照组,差异均有统计学意义(P<0.05)。结论:来曲唑片联合益母草颗粒治疗PCOS患者,可有效提高临床疗效,改善患者性激素水平、糖脂代谢异常和卵泡凋亡情况。OBJECTIVE: To probe into the clinical efficacy of Letrozole tablets combined with Leonurus granules in the treatment of polycystic ovary syndrome(PCOS) and its effects on the expression of peripheral blood transcription factor FOXO1, serum amyloid protein P(SAP) and death associated protein kinase 1(DAPK1). METHODS: Totally 154 patients with PCOS admitted into Specialty Medical Center of Chinese People’s Armed Police Force from Aug. 2017 from Mar. 2020 were selected and divided into control group(77 cases) and treatment group(77 cases) via random number table. Patients in the control group took Letrozole tablets orally on the 5 th day of menstruation, 5 mg a time and once a day, 5 days for 1 course of treatment. Patients in the treatment group took Leonurus granules orally based on the control group, 15 g a time and twice a day, from the 5 th day of menstruation until the next menstruation for 1 course of treatment;both groups were given continuous 3 courses of treatment. The clinical efficacy, sex hormone and glucose metabolism levels, the expression of FOXO1, SAP and DAPK1 before and after treatment were compared between two groups. RESULTS: The total effective rate of treatment group was 98.70%(76/77), which was higher than 90.91%(70/77) in the control group, with statistically significant difference(P<0.05). Compared with before treatment, the sex hormone(serum luteinizing hormone, follicle-stimulating hormone and testosterone) levels of both groups were decreased after treatment, the sex hormone levels of the treatment group after treatment were lower than those of the control group, with statistically significant differences(P<0.05). Compared with before treatment, the expresion levels of fasting insulin(FINS), insulin-like growth factor 1(IGF-1) and insulin resistance index(HOMA-IR) of both groups were decreased after treatment, the expresion levels of FINS, IGF-1 and HOMA-IR of the treatment group after treatment were lower than those of the control group, with statistically significant differences(P<0.
