摘 要:皮瓣移植是临床上治疗皮肤破损创面的一种常用手段,目前常采用样布描记法确定皮瓣形状.由于创面的表面形态复杂,获得的皮瓣轮廓精度低,术后愈合效果差.为此,提出一种基于变形能的皮肤创面展开方法.首先,应用层次值对创面模型进行层次划分,将创面形态学展开至平面;然后,引入有限单元法,赋予创面网格模型中的三角单元以材料属性,定义单元由空间展开至平面产生的变形能,模拟皮瓣在覆盖创面时可能发生的变形;最后,根据变形能优化调整展开形状实现对皮瓣形状的区域优化与术前设计.选取十几例复杂创面模型进行展开分析的结果表明,所提方法优化后的面积误差可缩小至5.63%以内,且变形能向最小稳定值收敛.Skin flap grafting is a common clinical means for the treatment of broken skin wounds,and the shape of the flap is often determined by sample cloth for preoperative design.Due to the complex surface morphology of the wound,obtained flaps have low precision and poor postoperative healing results.A deformation energy-based flattening method for skin wounds is proposed.Firstly,the wound model is divided into layers by hierarchical value and flattened to a plane topologically.The material properties are then assigned to triangle elements on the wound model by finite element method,and defines the deformation energy of the element during flattening,simulating the deformation when the flap covers the wound.Finally,the shape of the flap can be adjusted by deformation energy to achieve regional optimization of the flap shape and preoperative design.The results of analyses of a dozen complex wound models showed that the optimized method could reduce the area error to within 5.63%and the deformation energy could converge to the minimum stable value.
