伤骨再生汤联合双切口金属植入物内固定治疗胫骨平台双髁骨折的临床疗效

摘　　要：目的探讨伤骨再生汤联合双切口金属植入物内固定法治疗胫骨平台双髁骨折患者的临床疗效。方法选取2019年5月至2020年5月本院收治的88例胫骨平台双髁骨折患者作为研究对象,按随机抽签法分为观察组和对照组,每组44例。对照组采用双切口金属植入物内固定法治疗,观察组在对照组基础上联合伤骨再生汤辅助治疗,比较两组临床疗效、Rasmussen放射学评分、Rasmusse膝关节功能评分、血清Ⅰ型胶原交联羧基末端肽(CTX)和Ⅰ型前胶原氨基端前肽(PINP)、血清炎症因子[白细胞介素-5(IL-5)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)]水平及并发症发生情况。结果观察组治疗总有效率(90.91%)高于对照组(72.73%),差异有统计学意义(P<0.05)。治疗后,两组Rasmussen放射学评分、Rasmusse膝关节功能评分均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清中CTX、PINP水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清IL-5、IL-6、IL-10水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率(4.55%)低于对照组(18.18%),差异有统计学意义(P<0.05)。结论伤骨再生汤联合双切口金属植入物内固定治疗胫骨平台双髁骨折效果显著,可有效改善患者膝关节功能及血清炎症因子水平,降低并发症发生率,安全性较高,具有较高的临床应用价值。Objective To investigate the clinical efficacy of Shanggu regeneration decoction combined with double incision metal implant internal fixation in the treatment of bicondylar fracture of tibial plateau.Methods 88 patients with bicondylar fracture of tibial plateau were treated in our hospital from May 2019 to May 2020 were selected as the research subjects,and they were divided into observation group and control group according to random drawing method,with 44 cases in each group.The control group was treated with double incision metal implant internal fixation and the observation group was treated with Shanggu regeneration decoction on the basis of the control group,the clinical efficacy,Rasmussen radiology score,Rasmusse knee function score,serum typeⅠcollage cross-linked-telopeptide(CTX)and procollagen typeⅠN-terminal propeptide(PⅠNP)of,serum inflammatory factors(interleukinn-5[IL-5],interleukin-6[IL-6],interleukin-10[IL-10])level and incidence of complicationswere compared between the two groups.Results The total effective rate of the observation group(90.91%)was significantly higher than(72.73%)of the control group,and the difference was statistically significant(P<0.05).After treatment,the Rasmussen radiology score and Rasmusse knee joint function score in the two groups were higher than before treatment,and the observation group was higher than the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of serum CTX and PINP in the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of serum IL-5,IL-6 and IL-10 in two groups were lower than before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group(4.55%)was lower than(18.18%)in the control group,and the difference was statistically significant
