血清HIV抗体筛查阳性与免疫蛋白印迹试验确认结果分析

摘　　要：目的分析血清HIV抗体筛查阳性结果与免疫蛋白印迹试验的结果,探讨确证结果与抗体筛查阳性结果间的关系。方法选取2017年8月至2019年12月本中心HIV抗体初步筛查阳性的246例患者作为研究对象,对所有患者的血清结果进行HIV抗体筛查阳性复检,应用免疫蛋白印迹试验检测复检阳性患者血清行确证试验。结果初筛阳性246例标本中,经免疫蛋白印迹试验确认,有212例HIV抗体阳性,阴性21例,不确定13例。经免疫蛋白印迹法确认后,复检结果1≤S/CO<6、6≤S/CO<10、S/CO≥10与免疫蛋白印迹确认阳性符合率分别为63.41%、83.54%、95.24%。免疫蛋白印迹试验确认的212例HIV-1型抗体阳性中,gp160、gp120、gp41、p66、p51、P24条带的出现率均>92%,且gp160、gp120的出现率为100%。结论血清HIV抗体筛查存在假阳性结果,需进一步行确证试验,同时应保证血液质量,避免出现假阳性结果;随着S/CO值的升高,与确证试验的阳性符合率升高,可为艾滋病的预防控制工作提供及时、准确的科学参考,但免疫蛋白印迹试验也存在不确定结果。Objective To analyze the positive results of serum HIV antibody screening and the results of Western blot test,and to explore the relationship between the confirmed results and the positive results of antibody screening.Methods A total of 246 patients with preliminary HIV antibody screening positive in the center from August 2017 to December 2019 were selected as the research subject.The serum results of all patient were retested for HIV antibody screening positive,and the sera of patients with retest positive were performed by Western blot test.Results Among the 246 positive samples,212 were HIV antibody positive,21 were negative,and 13 were uncertain.After confirmed by Western blot,the coincidence rates of 1≤S/CO<6,6≤S/CO<10,S/CO≥10 with Western blot were 63.41%,83.54%and 95.24%,respectively.Western blot test confirmed the result of HIV-1 antibody positive in 212 cases,the occurrence rates of gp160,gp120,gp41,p66,p51,and p24 bands were>92%,and the occurrence rates of gp160 and gp120 were 100%.Conclusion There are false positive results of serum HIV antibody screening,which needs to be confirmed by a walk test.At the same time,the blood quality should be guaranteed to avoid false positive results;with the increase of S/CO value,the positive coincidence rate with the confirmatory test is also increasing,can provide timely and accurate scientific reference for AIDS prevention and control,but there is also an uncertain result in immunoblotting test.
