肺泡灌洗液中IL-8水平对难治性肺炎支原体肺炎患儿的诊断及疗效评估价值分析

摘　　要：目的探究肺泡灌洗液中白细胞介素-8(IL-8)水平对难治性肺炎支原体肺炎(RMPP)患儿的诊断及疗效评估价值。方法选取120例RMPP患儿作为观察组,另选同期普通型肺炎支原体肺炎(MPP)患儿50例作为对照组,两组患儿进行常规对症治疗及支气管肺泡灌洗。比较两组患儿肺泡灌洗液中IL-8水平,并分析IL-8诊断价值;分析观察组患儿治疗效果,并比较不同疗效患儿肺泡灌洗液中IL-8水平;分析观察组治疗显效患儿肺泡灌洗液中IL-8的评估价值。结果第1次治疗时,观察组肺泡灌洗液中IL-8水平为(15.58±2.73)pg/ml,高于对照组的(13.11±2.08)pg/ml,差异具有统计学意义(P<0.05);采用IL-8诊断RMPP的AUC为0.749,Cutoff值为14.001 pg/ml。疗程结束时,观察组中显效90例、有效27例、无效3例,患儿肺泡灌洗液中IL-8水平分别为(8.41±1.06)、(10.25±1.48)、(11.84±0.73)pg/ml,比较差异有统计学意义(P<0.05)。疗程结束时,肺泡灌洗液中IL-8判断RMPP治疗显效的AUC为0.881,Cutoff值为9.848 pg/ml。结论肺泡灌洗液中的IL-8水平能够作为诊断RMPP及评估疗效的有效指标。Objective To investigate the value of interleukin-8(IL-8)level in bronchoalveolar lavage fluid in diagnosis and efficacy evaluation of children with refractory Mycoplasma pneumoniae pneumonia(RMPP).Methods 120 children with RMPP were selected as the observation group,and 50 children with ordinary Mycoplasma pneumoniae pneumonia(MPP)during the same period were selected as the control group.Both groups received conventional symptomatic treatment and bronchoalveolar lavage.The levels of IL-8 in the bronchoalveolar lavage fluid of the two groups were compared,and the diagnostic value of IL-8 was analyzed.The therapeutic effect of the observation group was analyzed,and the levels of IL-8 in the bronchoalveolar lavage fluid of the children with different efficacy were compared.The evaluation value of IL-8 in bronchoalveolar lavage fluid of children with markedly effective treatment in observation group was analyzed.Results At the 1st treatment,the level of IL-8 in the bronchoalveolar lavage fluid of the observation group was(15.58±2.73)pg/ml,which was higher than(13.11±2.08)pg/ml of the control group,and the difference was statistically significant(P<0.05).Using IL-8 to diagnose RMPP,the AUC was 0.749,and the Cutoff value was 14.001 pg/ml.At the end of the course,90 cases were markedly effective,27 cases were effective,and 3 cases were ineffective in the observation group,and the IL-8 levels in the alveolar lavage fluid were(8.41±1.06),(10.25±1.48),and(11.84±0.73)pg/ml,and the differences were statistically significant(P<0.05).At the end of the course,the AUC of IL-8 in the alveolar lavage fluid to determine the efficacy of RMPP treatment was 0.881 and the Cutoff value was 9.848 pg/ml.Conclusion The level of IL-8 in bronchoalveolar lavage fluid can be used as an effective index for the diagnosis and evaluation of RMPP.
