摘　　要：目的探讨采用简单内固定辅助铰链式外固定支架治疗肱骨远端冠状面骨折的疗效。方法2017年1月至2019年4月共收治肱骨远端冠状面骨折患者12例,受伤时间2 h~4个月(平均15.2 d),采用切开复位空心钉、克氏针等简单内固定方式辅助肘关节铰链式外固定支架进行治疗,采用Mayo肘关节功能评分对其疗效进行评定。结果所有患者获得6~19个月(平均12.7个月)随访,肱骨远端骨折全部愈合,临床愈合时间2.5~4个月,平均3.6个月。所有患者术后均未发生肘关节僵硬、晚期缺血性骨坏死及骨吸收和异位骨化等并发症。患者末次随访时,肘关节平均活动范围伸直12.2°±4.6°,屈曲118.2°±12.6°,前臂旋前82.6°±6.7°,旋后69.4°±6.5°。按照Mayo肘关节功能评分标准,优8例,良3例,可1例,优良率为91.7%。结论采用切开复位简单内固定方式辅助肘关节铰链式外固定支架治疗肱骨远端冠状面骨折,术后可早期进行功能锻炼,有利于肘关节功能恢复,疗效满意。Objective To evaluate the treatment results of a hinged external fixator in combination with internal fixation for coronal shear fractures of the distal end of the humerus.Methods Twelve cases of coronal shear fractures of the distal end of the humerus treated in our department were studied retrospectively.All the fractures were treated with a hinged external fixator in combination with internal fixation and evaluated according to the Mayo elbow performance scores.Results Twelve patients were followed up for 6 to 19 months(mean,12.7 months).The union time was 3.6 months(ranging from 2.5 to 4 months).The final range of flexion to extension of the elbow was 12.2°±4.6°to 118.2°±12.6°.The final ranges of pronation and supination were 82.6°±6.7°and 69.4°±6.5°.Based on Mayo elbow performance scores,post-operative elbow functional outcomes of eight patients were excellent,three good,and one fair.Conclusion Coronal shear fractures of the distal end of the humerus can be effectively treated with a hinged external fixator in combination with internal fixation,facilitating early rehabilitation and restoration of elbow function.
