摘 要:目的:探讨血清PA、NT-proBNP与25羟维生素D_(3)、25-(OH)D_(3)水平对川崎病患儿冠状动脉病变风险的预测价值。方法:回顾性分析的96名健康体检者,设为对照组;选取98例川崎病患儿为观察组,根据超声心电图检查结果分为扩张组(冠状动脉病变,n=41),无扩张组(无冠状动脉病变,n=57)。比较3组对象N端脑肽前体(NT-proBNP)、前白蛋白(PA)、25-(OH)D_(3)水平,分析影响冠状动脉病变的因素及NT-proBNP、PA、25-(OH)D_(3)水平对冠状动脉病变风险的预测效能。结果:观察组患者NT-proBNP、25-(OH)D_(3)水平高于对照组(P<0.05);PA低于对照组(P<0.05)。扩张组NT-proBNP、25-(OH)D_(3)水平高于无扩张组(P<0.05);PA低于无扩张组(P<0.05)。回归分析显示,25-(OH)D_(3)是冠状动脉病变的危险因子(P<0.05);PA是冠状动脉病变的保护因子(P<0.05)。ROC曲线分析,NT-proBNP、25-(OH)D_(3)、PA及三项指标联合预测冠状动脉病变风险的曲线下面积(AUC)分别为0.871、0.890、0.919、0.977。结论:血清PA、NT-proBNP与25-(OH)D_(3)水平对川崎病病儿冠状动脉病变发生风险具有一定效能。Objective:To explore the predictive value of serum PA,NT-proBNP and 25-(OH)D_(3)levels on the risk of coronary artery disease in children with Kawasaki disease.Methods:A retrospective analysis of 96 healthy people served as the control group,98 children with Kawasaki disease were selected as the observation group.According to the results of ultrasonic ECG,they were divided into dilation group(coronary artery disease,n=41)and non dilation group(no coronary artery disease,n=57).The N-terminal brain peptide precursor(NT-proBNP),prealbumin(PA),25-hydroxyvitamin D_(3)[25-(OH)D_(3)]were compared between the three groups.The factors affecting coronary artery disease and the predictive efficacy of NT-proBNP,PA and 25-(OH)D_(3)levels on the risk of coronary artery disease were analyzed.Results:The levels of NT-proBNP and 25-(OH)D_(3)in the observation group were higher than those in the control group,and PA in the observation group was lower than that in the control group(P<0.05).The levels of NT-proBNP and 25-(OH)D_(3)in the dilation group were higher than those in the non-dilation group,and PA in the dilation group was lower than that in the non dilation group(P<0.05).Regression analysis showed that 25-(OH)D_(3)was a risk factor for coronary artery disease,PA were protective factors for coronary artery disease(P<0.05).ROC curve analysis showed that the AUC of NT-proBNP,25-(OH)D_(3),PA,and the three indicators combined to predict the risk of coronary artery disease were 0.871,0.890,0.919,and 0.977,respectively.Conclusion:Serum levels of PA,NT-proBNP and 25-(OH)D_(3)have certain effects on the risk of coronary artery disease in children with Kawasaki disease.
