自拟宣肺化痰汤治疗老年慢性支气管炎急性发作的远期疗效及安全性

摘　　要：目的:分析老年慢性支气管炎(CB)急性发作期患者应用自拟宣肺化痰汤治疗的疗效。方法:将104例老年CB患者按照治疗方法不同分为研究组和对照组,每组各52例。对照组给予左氧氟沙星+氨茶碱片治疗,研究组在对照组的基础上增服自拟宣肺化痰汤。比较两组治疗前后用力肺活量(FVC)、第一秒用力呼气量(FEV1)及每分钟最大通气量(MVV)、中医症候积分、白介素10(IL-10)和肿瘤坏死因子-α(TNF-α)水平,统计两组急性发作次数及不良反应发生率。结果:治疗后,两组FEV1、FVC、MVV水平均较治疗前升高,研究组FEV1、FVC、MVV水平均高于对照组(P<0.05);两组咳嗽、咳痰和喘息积分均降低,研究组症候积分低于对照组(P<0.05);研究组临床总有效率高于对照组(P<0.05);两组IL-10和TNF-α水平均降低,研究组IL-10和TNF-α水平低于对照组(P<0.05);研究组急性发作次数较对照组减少(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:自拟宣肺化痰汤结合常规西医治疗可有效改善老年CB患者急性发作期的肺功能,缓解临床症状,减少远期急性发作次数,临床应用价值较高。Objective:To analyze the efficacy of self-made xuanfei huatan decoction in elderly patients with acute attack of chronic bronchitis(CB).Methods:104 elderly patients with CB were divided into study group(n=52)and control group(n=52)according to the difference of treatment methods.The control group was treated with levofloxacin and aminophylline tablets,and the research group was additionally administrated with Xuanfei Huatan Decoction on the basis of the control group.The forced vital capacity(FVC),forced expiratory volume in one second(FEV1)and maximal voluntary ventilation(MVV),TCM symptom score,interleukin-10(IL-10)and tumor necrosis factor-α(TNF-α)levels before and after treatment were compared between the two groups.The number of acute episodes and the incidence of adverse reactions were counted between the two groups.Results:After treatment,the levels of FEV1,FVC and MVV in both groups were higher than those before treatment,and the levels of FEV1,FVC and MVV in the study group were higher than those in the control group(P<0.05).After treatment,the cough,expectoration and wheeze scores of the two groups were decreased,and the symptom scores of the research group were lower than those of the control group(P<0.05).The total clinical effective rate in the treatment group was higher than that in the control group(P<0.05).The levels of IL-10 and TNF-αin the two groups were decreased,and the levels of IL-10 and TNF-αin the research group were lower than those in the control group(P<0.05).The number of acute episodes in the treatment group was less than that in the control group(P<0.05),and there was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The treatment with self-made Xuanfei Huatan Decoction is safe and effective in improving lung function of elderly CB patients in the acute attack phase,alleviating clinical symptoms and reducing the number of long-term acute attacks,and it is worthy of promotion.
