眼睑黄色瘤1104例患者血脂7项的调查分析

摘　　要：目的对1104例眼睑黄色瘤患者的血脂7项进行调查与研究。方法采用整群抽取的方法,对被调查者进行眼睑黄色瘤筛查和空腹血清三酰甘油(triacylglycerol,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low-density lipoprotein-cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C),载脂蛋白-A1(apolipoprotein-A1,Apo-Al),载脂蛋白-B(apolipoprotein-B,Apo-B),脂蛋白(a)[lipopro⁃tein(a),Lp(a)]检测。结果眼睑黄色瘤发病率为2.58%(1104/42691),女性(3.09%)高于男性(2.04%),差异有统计学意义(χ^(2)=46.82,P<0.01)。血脂异常发病率为39.76%(16972/42691),男性(45.59%)高于女性(34.35%),差异有统计学意义(χ^(2)=562.29,P<0.01)。Logistic回归分析结果显示,眼睑黄色瘤与血脂7项相关:TG(OR=23.007)、TC(OR=23.976)、HDL-C(OR=13.016)、LDL-C(OR=22.395)、Apo-Al(OR=11.471)、Apo-B(OR=17.193)、Lp(a)(OR=89.786)。结论眼睑黄色瘤的发生与血脂7项的异常相关,尤其与Lp(a)的异常密切相关。Objectives To investigate the 7 items of blood lipid in 1104 patients with eyelid xanthoma.Methods The cluster extraction method was used.The subjects were screened for eyelid xanthoma and their fasting blood lipid including serum concentrations of triglycerides(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein-A1(Apo-Al),apolipoprotein-B(Apo-B),lipoprotein(a)[Lp(a)]were tested.Results Prevalence of eyelid xanthoma was 2.58%(1104/42691),which in female(3.09%)was higher than that in male(2.04%),and the difference was statistically significant(χ^(2)=46.82,P<0.01).Prevalence of dyslipidemia was 39.76%(16972/42691),which in male(45.59%)was higher than that in female(34.35%),and the difference was statistically significant(χ^(2)=562.29,P<0.01).Logistic regression analysis indicated that there were positive correlations between eyelid xanthoma and 7 items of blood lipid:TG(OR=23.007),TC(OR=23.976),HDL-C(OR=13.016),LDL-C(OR=22.395),Apo-Al(OR=11.471),Apo-B(OR=17.193),Lp(a)(OR=89.786).Conclusions The occurrence of eyelid xanthoma associates with dyslipidemia.Especially,it is closely related to the abnormality of LP(a).
