摘　　要：目的对成人ICU患者转出时主要照顾者的迁移应激压力水平进行调查,并明确影响因素。方法2019年1—2月采用方便抽样方法选取厦门市某三级甲等综合性医院126名待转出ICU的成人患者的主要照顾者进行应激压力水平问卷调查。结果ICU患者主要照顾者应激压力总分(51.08±6.84)分,照顾能力的自我效能感、转出过程安全性及病情危重程度三个维度得分最高。多因素分析结果显示,患者所在科室及入ICU次数是照顾者迁移应激水平的独立影响因素(P<0.05)。结论主要照顾者在患者转出时的应激压力水平为中等。科室及入ICU次数是照顾者迁移应激水平的独立影响因素,医务人员应关注重点人群的内心体验,了解其转出前的心理状况,对其不安情绪表示理解并针对性引导;同时引导照顾者参与到患者康复过程中,帮助其顺利度过心理应激期。Objective Investigated the migration stress level of primary caregivers when adult intensive care unit(ICU)patients were transferred out,and identified the influencing factors.Methods From January to February 2019,the main caregivers of 126 adult patients who were to be transferred out of the ICU in a tertiary and firstclass general hospital in Xiamen were selected by the convenience sampling method to conduct a questionnaire survey on the stress level.Results The total stress score of the main caregivers of ICU patients was(51.08±6.84)points,and the three dimensions of selfefficacy of caring ability,safety of the transfer process and the degree of critical illness were the highest.The results of multivariate analysis showed that the patient's department and the number of ICU admissions were independent influencing factors of caregivers'migration stress level(P<0.05).Conclusion The stress level of the primary caregiver when the patient was transferred out was moderate.The patient's department and the number of ICU admissions were independent influencing factors of caregivers'migration stress level.Medical staff should pay attention to the inner experience of key populations,understand their psychological conditions before being transferred out,and understand their anxiety.and targeted guidance;at the same time,guide caregivers to participate in the patient’s rehabilitation process to help them pass through the psychological stress period smoothly.
