潜剥虚挂法联合内口袋形缝合术治疗高位肛瘘48例疗效观察

摘　　要：目的:观察潜剥虚挂法联合内口袋形缝合术治疗高位肛瘘的疗效。方法:将高位肛瘘患者93例分为对照组45例和观察组48例,对照组患者采用常规内口上方实挂法治疗,观察组患者采用潜剥虚挂法联合内口袋形缝合术治疗,比较2组患者的疗效、术后2、4、6d疼痛评分、瘢痕面积、术后大便失禁严重度评分(Wexner评分)及创面愈合时间。结果:2组患者总有效率比较差异无统计学意义(P>0.05);观察组患者术后2d、4d、6d疼痛评分均低于对照组(P<0.05);观察组患者瘢痕面积小于对照组(P<0.05),Wexner评分低于对照组(P<0.05),创面愈合时间短于对照组(P<0.05)。结论:潜剥虚挂法联合内口袋形缝合术治疗高位肛瘘患者具有疼痛轻、损伤小、创面愈合时间快等优点。Objective To observe the effect of high anal fistula surgically treated with the combined procedure of potentially dissection&suspension thread-drawing and bag-like suture for internal opening.Methods Divided 93 patients with high anal fistula into control group(45 cases,received routine really thread-drawing over internal opening)and observation group(48 cases,received above-mentioned the combined procedure),then,compared both gorups’effect;the ratings on pain at 2,4,and 6 days after surgery,scar area,and postoperative Wexner score ratings,as well as wound-surface healing time.Results In total effective rate there was no statistical difference between the two groups(P>0.05),but,in the ratings on pain at 2,4,6 days after surgery,in scar area,in Wexner ratings,and in the time for wound-surface healing,observation group was respectively lower,smaller,lower,and shorter than control group(P<0.05,all).Conclusion The combined procedure used in observation group for high anal fistula has such merits as mild pain,little trauma,and shorter wound-surface healing,etc.
