摘　　要：目的分析非肌层浸润性膀胱癌患者在行经尿道膀胱肿瘤切除术(TURBT)后发生膀胱痉挛的影响因素,为其防治提供依据。方法回顾性收集228例行TURBT的非肌层浸润性膀胱癌患者的临床资料,按术后是否发生膀胱痉挛分为痉挛组和未痉挛组,分析2组患者性别、年龄、术前尿路感染、肿瘤数量、手术时间、手术次数、肿瘤位置等方面的差异。结果单因素分析结果显示,术前尿路感染、肿瘤数量、手术时间、手术次数与TURBT后发生膀胱痉挛有关(P<0.05)。多因素分析结果显示,术前存在尿路感染、肿瘤数量多、手术时间长为TURBT后发生膀胱痉挛的危险因素(P<0.05)。结论行TURBT的非肌层浸润性膀胱癌患者中,术前存在尿路感染、肿瘤数量多、手术时间长是术后发生膀胱痉挛的危险因素,术前可结合患者的情况,给予针对性干预措施,促进患者术后恢复。Objective To analyze the significant factors associated with bladder spasms after transurethral resection of bladder tumor(TURBT)in patients with non-muscle invasive bladder cancer,in order to provide evidence for their prevention and treatment.Methods The clinical data of 228 patients with non-muscle invasive bladder cancer,who had undergone TURBT,were collected retrospectively.The patients were divided into a spasm group in which bladder spasms had occurred after their operations,and a nonspasm group.Differences in sex,age,preoperative urinary tract infections,tumor numbers,operation durations,number of operations,and tumor locations were analyzed.Results Univariate analysis showed that preoperative urinary tract infections,tumor numbers,operation durations,and number of operations were related to bladder spasms after TURBT(P<0.05).Multivariate analysis indicated that preoperative urinary tract infections,a large number of tumors,and a long operation duration were risk factors for bladder spasms after TURBT(P<0.05).Conclusion Preoperative urinary tract infections,tumor numbers,and operation duration are related to bladder spasms in patients with non-muscle invasive bladder cancer after TURBT.Preoperative intervention measures can be taken in accordance with patients’conditions to promote postoperative recovery.
