郑永科运用小半夏加茯苓汤验案举隅

摘　　要：文章主要介绍郑永科辨证运用经方小半夏加茯苓汤治疗痰饮的经验。郑永科是德阳市首届十大名中医,工作50余年,临证经验丰富,医理纯熟,厚德仁心,遣方用药注重辨证,尤擅运用张机(字仲景)之经方治疗各种疾病,常收显效。小半夏加茯苓汤出自张仲景所著《金匮要略》。该方由茯苓、半夏、生姜组成,原文记载其主症涉及呕、吐、悸、眩、痞等。郑老认为使用经方必须辨证无误,方证结合严密,才能达到预期目的。小半夏加茯苓汤药味精简,配伍严谨,临床须辨证准确、找准病机、加减灵活,方可效如桴鼓。郑老常以此方为基础,温阳化饮,治疗饮停心下胃脘证偏寒者。在用药剂量方面,其法度严谨,分量亦有考究。笔者有幸师承郑老,侍诊多年,获益匪浅。现将郑老运用小半夏加茯苓汤的两则验案总结如下。This paper introduces famous doctor ZHENG Yongke’s experience in treating phlegm with the Xiaobanxia Fuling decoction(小半夏加茯苓汤).ZHENG Yongke is one of the first ten famous TCM doctors in Deyang City,who has been working for more than 50 years,and is rich in clinical experience.He is profound in the theory of TCM and has excellent techniques,and he has a generous moral heart and a benevolent attitude towards people.He is especially good at applying Zhongjing’s prescriptions to treat various diseases,and often achieves remarkable effects in his treatment.The Xiaobanxia Fuling decoction comes from Jingui Yaolue(《金匮要略》)written by ZHANG Zhongjing.It is composed of tuckahoe,pinellia and ginger.The main symptoms of the disease are vomiting,spitting,palpitations,dizziness and lump.Mr.ZHENG believes that the use of this prescription must be correct with accurate combination to achieve the desired purpose.It has simple taste and strict compatibility,so clinical differentiation must be accurate,identify the pathogenesis,flexible addition and subtraction before it can be effective.Mr.ZHENG often uses this prescription to nourish Yang(阳),the treatment can help the epigastric syndrome of cold.In terms of dosage,it has rigorous weight,which is also traceable.The author has the honor to learn from Dr.ZHENG for many years and has benefited a lot.Two successful cases of Dr.ZHENG in applying the Xiaobanxia Fuling decoction are summarized as follows.
