免疫组化法评估宫颈癌及宫颈上皮内瘤变组织中Survivin和mmp-9表达的临床意义

摘　　要：目的探索免疫组化法评估宫颈癌及宫颈上皮内瘤变组织中Survivin和mmp-9表达的临床意义。方法回顾性分析的49例CIN患者(对照组)、31例宫颈癌患者(观察组)均在2016年3月至2021年3月期间收集,均采用免疫组化法评估mmp-9、Survivin阳性率和染色积分,比较各组mmp-9、Survivin阳性率及染色积分,经相关性分析不同病变类型与Survivin、mmp-9染色积分相关性以及两项指标之间相关性。结果观察组Survivin阳性率64.52%、mmp-9阳性率77.42%、mmp-9染色积分(5.62±0.37)、Survivin染色积分(4.57±0.42)高于对照组患者(P<0.05)。CIN分级中,CINⅢ级mmp-9阳性率55.56%、Survivin阳性率44.44%、mmp-9染色积分(3.31±0.18)、Survivin染色积分(3.28±0.33)高于CINⅠ级(P<0.05)。不同临床分期、病理分级、淋巴结转移情况宫颈癌患者比较,Survivin、mmp-9阳性率存在差异性(P<0.05)。同时经Pearson法分析,Survivin染色积分与mmp-9染色积分呈正相关性,经Spearman法分析,不同病变类型与Survivin、mmp-9染色积分呈正相关性(P<0.05)。结论宫颈癌和宫颈上皮内瘤变组织均存在Survivin、mmp-9蛋白表达升高,宫颈癌蛋白表达情况与病理分级、临床分期、淋巴结转移情况相关。Objective To investigate the clinical significance of Survivin and MMP-9 expression in cervical cancer and cervical intraepithelial neoplasia by immunohistochemical method.Methods In retrospective analysis,49 CIN patients(control group)and 31 cervical cancer patients(observation group)were collected from March 2016 to March 2021,and MMP-9 and Survivin positive rates and staining scores were evaluated by immunohistochemistry,and MMP-9 and Survivin positive rates and staining scores were compared among each group.The correlation between Survivin and MMP-9 staining scores and the two indexes were analyzed.Results The Survivin positive rate(64.52%),MMP-9 positive rate(77.42%),MMP-9 staining score(5.62±0.37)and Survivin staining score(4.57±0.42)in the observation group were higher than those in the control group(P<0.05).In CINⅢ grade,MMP-9 positive rate(55.56%),Survivin positive rate(44.44%),MMP-9 staining score(3.31±0.18),Survivin staining score(3.28±0.33)were higher than CINⅠ grade(P<0.05).The positive rates of Survivin and MMP-9 were different among cervical cancer patients with different clinical stages,pathological grades and lymph node metastasis(P<0.05).The Survivin staining score was positively correlated with MMP-9 staining score by Pearson's method,and the Survivin staining score was positively correlated with MMP-9 staining score by Spearman's method(P<0.05).Conclusion The expression of Survivin and MMP-9 protein is increased in cervical cancer and cervical intraepithelial neoplasia.The expression of cervical cancer protein is related to pathological grade,clinical stage and lymph node metastasis.
