基于加速康复外科理念的品管圈在颈动脉内膜剥脱术围手术期中的应用效果

摘　　要：目的探讨基于加速康复外科(ERAS)理念的品管圈(QCC)活动在颈动脉内膜剥脱术(CEA)围手术期中的应用效果。方法成立课题研究型QCC活动小组,通过现况调查、攻坚点的发掘,明确课题,设定目标,拟定方策,进行最适方策探究,形成方策群组并实施,实施后进行效果评价。结果开展QCC活动后,神经外科医护人员对ERAS知识认知水平显著提高(P<0.05);行CEA手术患者术后拔尿管时间及首次下地时间显著缩短(P<0.05);行CEA手术患者术后住院日显著缩短(P<0.05),圈员QCC手法应用、责任感、凝聚力、沟通配合能力及解决问题的能力显著提高。结论开展QCC活动可提高医护人员ERAS知识水平,加速行颈动脉内膜剥脱术患者的康复进程,促进护理质量的持续改进。Objective To investigate the effect of quality control circle(QCC)based on enhanced recovery surgery(ERAS)concept in perioperative period of carotid endarterectomy(CEA).Methods Set up a research QCC activity group for the"people and circle"subject,through the current situation investigation and the discovery of key points,clarify the subject,set the goal,formulate the strategy,explore the most suitable strategy,form a strategy group and implement it,and after implementation Conduct an effect evaluation.Results After the QCC activities were carried out,the level of medical staff's knowledge of ERAS was significantly improved(P<0.05),the time for the patient to remove the catheter and the first time to go to the ground were significantly shortened(P<0.05),and the postoperative hospital stay was significantly shortened(P<0.05),the circle members'QCC technique application,sense of responsibility,cohesion,communication and cooperation ability and problem-solving ability are significantly improved.Conclusion The development of QCC activities can improve the knowledge level of ERAS of medical staff,accelerate the recovery of patients undergoing carotid endarterectomy,and promote continuous improvement of nursing quality.
