摘 要:目的探讨MP-IgM、MP-IgG抗体以及MP-DNA联合检测诊断儿童支原体肺炎的临床价值。方法将2019年1月至2019年10月我院收治的158例疑似肺炎支原体肺炎患儿设为试验组,将同期160名在我院进行保健检查的健康儿童设为正常对照组,均应用化学发光法(CLIA)检测MP-IgM、MP-IgG抗体,并采用荧光半定量PCR法检测MP-DNA,比较两组检测结果。结果试验组的MP-IgM、MP-IgG、MP-DNA阳性率均显著高于正常对照组(P<0.05)。在试验组中,就诊当日的MP-IgM阳性检出率为32.91%,在治疗2周后达高峰,随后逐渐降低;MP-IgG就诊当日阳性检出率为3.16%,在治疗4周后达高峰;MP-DNA就诊当日的阳性检出率为75.95%,随着病程的延长,其阳性率逐渐下降。上述3项指标的阳性检出率在不同病程比较差异均有统计学意义(P<0.05)。结论在肺炎支原体感染患儿不同病程时期应用化学发光法同时检测MP-IgM、MP-IgG抗体以及MP-DNA可实现优势互补,有效提高儿童支原体肺炎的诊断效能,减少漏诊、误诊情况的发生,对于本病早期诊治具有重要的临床意义。Objective To explore the clinical value of detecting MP-IgM,MP-IgG antibody and MP-DNA simultaneously in the diagnosis of mycoplasma pneumonia in children.Methods From January 2019 to October 2019,158 children suspected of mycoplasma pneumoniae pneumonia in our hospital were selected as the experimental group,and 160 healthy children who had health care examination in our hospital at the same time were selected as the normal control group.MP-IgM and MP-IgG antibodies were detected by CLIA,and MP-DNA was detected by fluorescence semi quantitative PCR.The results of the two groups were compared.Results The positive rates of MP-IgM,MP-IgG and MP-DNA in the experimental group were significantly higher than normal control group(P<0.05).The positive rate of MP-IgM in the experimental group was 32.91%on the day of treatment,which peaked after 2 weeks of treatment,and then gradually decreased.The positive rate of MP-IgG on the day of visit was 3.16%,and reached the peak after 4 weeks of treatment.The positive rate of MP-DNA was 75.95% on the day of visit.With the course of disease prolonged,the positive rate decreased gradually.The positive detection rate of three indexes in different course of disease were statistically significant(P<0.05).Conclusion The application of chemiluminescence in the detection of MP-IgM,MP-IgG antibody and MP-DNA in different course of mycoplasma pneumoniae infection can achieve complementary advantages,effectively improve the diagnosis of mycoplasma pneumoniae in children,and reduce the missed diagnosis and misdiagnosis of the disease.
