摘  要:目的探讨磁压榨技术(Magnetic Compression Technique,MCT)用于膀胱造瘘的可行性。方法根据MCT原理,结合新西兰兔下尿道解剖特点,设计用于膀胱造瘘的子磁体(Daughter Magent,DM)和母磁体(Parent Magnet,PM)。以6只新西兰兔为动物模型,静脉麻醉后经尿道置入DM至膀胱内,在耻骨联合上方腹壁外放置PM。DM和PM可自动相吸。密切观察实验兔术后一般状况,记录磁体脱落时间。磁体脱落后安乐死处死实验兔获取膀胱造瘘口标本,肉眼观察膀胱造瘘口形成情况。结果6只实验兔均成功实施了MCT膀胱造瘘,术中DM在导丝引导下顺利置入膀胱内,DM和PM自动对位吸合良好。术后10~13 d DM和PM脱落,膀胱造瘘口建立,开腹探查见膀胱与腹壁粘连愈合良好,盆腔无明显粘连。肉眼观察膀胱造瘘口标本可见膀胱与腹壁粘连愈合紧密,造瘘口通畅性良好。结论基于MCT的膀胱造瘘具有操作简便、创伤小、造瘘口形成良好等优点,该技术有望用于临床。Objective To investigate the feasibility of magnetic compression technique(MCT)for cystostomy.Methods The daughter magnet(PM)and parent magnets(PM)for cystostomy were designed according to the principle of MCT and the anatomical characteristics of the lower urethra of New Zealand rabbits.Six New Zealand rabbits were used as animal models.After intravenous anesthesia,the DM was inserted into the bladder through the urethra,and the PM was placed outside the abdominal wall above the symphysis pubis.The DM and PM can be automatically attracted.The general conditions of rabbits after operation were closely observed and the expulsion time of magnets was recorded.The rabbit was euthanized after the magnet fell off to obtain the fistula specimen and observe the formation of cystostomy by naked eye.Results Cystostomy by MCT was successfully performed in 6 rabbits.The DM was successfully inserted into the bladder under the guidance of the guide wire.Then the DM and PM were automatically aligned and attracted well.Ten to thirteen days after surgery,the DM and PM fell off and the cystostomy was established.Laparotomy showed that the adhesion between bladder and abdominal wall was well healed,and no obvious adhesion was found in pelvic cavity.The specimen of fistula was observed by naked eye,which showed that the bladder and abdominal wall adhered and healed closely and the fistula had good patency.Conclusion Cystostomy based on MCT has the advantages of simple operation,small trauma and good fistula formation,which is expected to be used in clinic.
