摘 要:目的研究慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)患者双期多层螺旋CT(Multi-Slice CT,MSCT)扫描参数与肺功能的相关性。方法回顾性选择2020年5月至2021年10月新疆喀什地区第一人民医院影像中心检查的160例COPD患者为COPD组,其中肺功能分级1级75例、2级54例、3级20例、4级11例,另选取同期40名健康体检者为对照组。采用格莱特Spirostik肺功能仪检查肺功能,西门子SOMATOM Definition Flash 64排螺旋CT行CT双期扫描检查。比较两组肺功能指标和CT双期扫描参数,用Pearson/Spearman相关性分析对COPD患者CT双期扫描参数与肺功能指标的相关性进行分析。绘制受试者工作特征(Receiver Operating Characteristic,ROC)曲线分析CT双期扫描参数对COPD患者肺功能分级的诊断价值。结果与对照组比较,COPD组肺活量(Vital Capacity,VC)、用力肺活量(Forced Vital Capacity,FVC)、一秒用力呼气容积(Forced Expiratory Volume in One Second,FEV_(1))、FEV_(1)/FVC、容积差均显著降低,残气量(Residual Volume,RV)、肺总量(Total Lung Capacity,TLC)、RV/TLC、深吸气末容积、深呼气末容积、容积比升高(P<0.05)。1级、2级、3级、4级COPD患者深吸气末容积、深呼气末容积、容积比依次升高,容积差依次降低(P<0.05)。Pearson/Spearman相关性分析显示,COPD患者深吸气末容积、深呼气末容积、容积比与VC、FVC、FEV_(1)、FEV_(1)/FVC呈负相关,与RV、TLC、RV/TLC呈正相关(P<0.05);容积差与VC、FVC、FEV_(1)、FEV_(1)/FVC呈正相关,与RV、TLC、RV/TLC呈负相关(P<0.05)。ROC曲线显示,CT双期扫描参数深吸气末容积、深呼气末容积、容积差、容积比对COPD患者肺功能分级具有一定诊断价值。结论COPD患者CT双期扫描参数深吸气末容积、深呼气末容积、容积比升高,容积差降低,这些参数与肺功能密切相关,可为COPD患者肺功能评价提供参考。Objective To study the correlation between dual-phase multi-slice CT(MSCT)parameters and pulmonary function in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 160 COPD patients examined in the imaging center of the Kashgar First People’s Hospital from May 2020 to October 2021 were retrospectively selected as the COPD group,including 75 cases of pulmonary function classification grade 1,54 cases of grade 2,20 cases of grade 3 and 11 cases of grade 4,and another 40 healthy people were selected as the control group during the same period.Pulmonary function was examined by Glatt Spirostik spirometer and dual-phase MSCT with a Siemens Somatom Definition Flash 64-slice spiral CT was performed.Pulmonary function indicators and dual-phase MSCT parameters between the two groups were compared,and the correlation between pulmonary function indicators and dual-phase MSCT parameters was analyzed by Pearson/Spearman correlation analysis in COPD patients.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of dual-phase CT scan parameters for the classification of pulmonary function in COPD patients.Results Compared with the control group,the COPD group’s vital capacity(VC),forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),FEV_(1)/FVC,volume difference all significantly decreased,while residual volume(RV),total lung capacity(TLC),RV/TLC,deep end-inspiratory volume,deep end-expiratory volume,and volume ratio all significantly increased(P<0.05).In COPD patients with pulmonary function grades 1,2,3,and 4,the deep end-inspiratory volume,deep end-expiratory volume,and volume ratio increased sequentially,and the volume difference decreased sequentially(P<0.05).Pearson/Spearman correlation analysis showed that deep end-inspiratory volume,deep endexpiratory volume and volume ratio were negatively correlated with VC,FVC,FEV_(1),FEV_(1)/FVC and positively correlated with RV,TLC,RV/TLC in COPD patients(P<0.05).The volume difference was positively cor
