摘　　要：目的探讨桃红四物汤加味联合运动疗法对肩袖损伤术后患者康复的疗效以及血清白细胞介素(IL)-6和肿瘤坏死因子(TNF)-α水平的影响。方法选取2017年5月-2020年5月在宜昌市第一人民医院肩关节镜下肩袖修补手术治疗的患者126例,按随机数字表法分为对照组和观察组均63例。对2组患者给予常规西医对症药物治疗,对照组术后进行系统运动疗法;观察组在对照组治疗基础上给予桃红四物汤加味治疗。比较2组肩关节功能和视觉模拟评分法(VAS)、肩关节活动度、临床疗效以及血清IL-6和TNF-α水平。结果治疗后,观察组肩关节功能ASES评分及肩关节的前屈、后伸、内旋、外旋各角度显著高于对照组,VAS评分明显低于对照组(P<0.01);观察组肩关节功能的优良率高于对照组(P<0.05);治疗后,观察组血清IL-6和TNF-α水平显著少于对照组(P<0.01)。结论桃红四物汤加味联合运动疗法对肩袖损伤术后患者的疗效明显,可有效缓解疼痛,促进肩关节功能改善,且可下调血清IL-6和TNF-α水平。Objective To investigate the curative effect of the modified Taohong Siwu Decoction combined with the exercise therapy on the postoperative rehabilitation of patients with a shoulder rotator cuffinjury,as well as its influence on the serum levels ofinterleukin-6(IL-6)and tumor necrosis factor-α(TNF-α).Methods A total of 126 patients who underwent a shoulder rotator cuffrepair surgery in Yichang First People’s Hospital from May 2017 to May 2020 were selected and randomly divided into a control group and an observation group,with 63 cases in each group.The two groups were both given a conventional western medicine symptomatic treatment,and the control group was additionally given a systemic exercise therapy after operation,while the observation group was treated with the Taohong Siwu Decoction on the basis of the control group.The scores of the shoulder joint function and visual analogue scale(VAS),the shoulder joint ranges of motion,clinical efficacy and the serum levels of IL-6 and TNF-αwere compared between the two groups.Results After treatment,the ASES score of the shoulder joint function and the angles of anterior flexion,posterior extension,internal rotation and external rotation of the shoulder joint in the observation group were higher than those in the control group,while the VAS score was significantly lower than that in the control group(P<0.01).The excellent and good rate of the shoulder function in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the serum levels of IL-6 and TNF-αin the observation group were significantly lower than those in the control group(P<0.01).Conclusion The modified Taohong Siwu Decoction combined with the exercise therapy is effective in the treatment of postoperative patients with a shoulder rotator cuffinjury.It can effectively relieve pains,improve the shoulder joint function,and down-regulate the serum levels of IL-6 and TNF-α.
