摘　　要：目的探究温针灸联合三伏贴对寒凝血瘀型膝关节骨性关节炎患者的治疗效果。方法选取2018年1月-2020年2月溧阳市人民医院中医科接诊的寒凝血瘀型膝关节骨性关节炎患者122例,随机分为温针灸组和联合治疗组。温针灸采用常规针刺合灸法,联合治疗组在其基础上加用三伏贴进行治疗,疗程为20 d。记录并比较2组患者治疗前后Lysholm、WOMAC及VAS评分及炎性反应因子、中医症候量化分级,疗程结束后评定治疗效果。结果2组患者治疗后Lysholm、WOMAC及VAS评分较治疗前均有改善,且联合治疗组改善更为显著(P<0.05);2组患者治疗后IL-1β、TNF-α、MMP-3炎性反应因子较治疗前均有降低,且联合治疗组下降更为显著(P<0.05);2组患者治疗后中医症候量化分级较治疗前均有下降,且联合治疗组下降更为显著(P<0.05);联合治疗组治疗总有效率(96.72%)显著高于温针灸组(85.25%)(P<0.05)。结论温针灸联合三伏贴治疗寒凝血瘀型膝关节骨性关节炎可显著改善患者临床症状,降低患者炎性因子水平,安全有效,值得临床推广。Objective To explore the therapeutic effect of warm acupuncture and moxibustion combined with Sanfu Patches on knee osteoarthritis patients with the syndrome of cold congelation and blood stasis.Methods From January 2018 to February 2020,122 knee osteoarthritis patients with the syndrome of cold congelation and blood stasis admitted to the department of traditional Chinese medicine of Liyang People’s Hospital were selected and randomly divided into a warm acupuncture and moxibustion group and a combined treatment group.The warm acupuncture and moxibustion group was treated with the conventional acupuncture therapy combined with moxibustion,while the combined treatment group was additionally treated with Sanfu Patchs,both for 20 days.The Lysholm,WOMAC and VAS scores,inflammatory response factors and quantitative gradings of TCM symptoms were recorded and compared between the two groups before and after treatment,and the treatment effect was evaluated after treatment.Results The Lysholm,WOMAC and VAS scores of the two groups were improved after treatment,and the improvement in the combined treatment group was more significant(P<0.05).Compared with before treatment,the levels of the IL-1β,TNF-α,and MMP-3 inflammatory response factors in both groups were decreased,and the decrease was more significant in the combined treatment group(P<0.05).The quantitative gradings of TCM syndromes in the two groups were decreased after treatment,and the decrease was more significant in the combined treatment group(P<0.05).The total effective rate of the combined treatment group(96.72%)was significantly higher than that of the warm acupuncture group(85.25%)(P<0.05).Conclusion In the treatment of knee osteoarthritis with the syndrome of cold congelation and blood stasis,warming acupuncture combined with Sanfu Patches can significantly improve clinical symptoms and reduce the level ofinflammatory factors in patients,which is safe and effective,and is worthy of clinical promotion.
