富血小板血浆联合点阵激光及米诺地尔治疗雄激素性脱发的临床疗效

摘　　要：目的:探讨富血小板血浆(PRP)联合点阵激光及米诺地尔治疗雄激素性脱发的临床疗效及安全性。方法:将30例雄激素性脱发患者随机分为对照1组(n=10),对照2组(n=10)和研究组(n=10),对照1组予以点阵激光与5%米诺地尔酊外涂治疗,对照2组予以单独PRP注射治疗,研究组予以PRP联合点阵激光与5%米诺地尔酊外涂治疗,治疗12周后评估疗效及安全性。结果:对照1组、对照2组和研究组三组痊愈例数均为0;对照1组显效2例,有效4例,无效4例;对照2组显效3例,有效4例,无效3例;研究组显效8例,有效1例,无效1例。经秩和检验,研究组疗效显著优于两对照组(P<0.05)。三组治疗后毳毛比例及单根毛发毛囊比例较治疗前均显著下降(P<0.05),毛发密度及毛干直径显著升高(P<0.05),且治疗后研究组的各项毛发镜指标均显著优于对照组(P<0.05)。研究组治疗后油腻、瘙痒、头屑及脱发症状较前显著改善(P<0.05)。治疗过程中三组均未出现严重不良反应。结论:富血小板血浆联合点阵激光及米诺地尔治疗雄激素性脱发疗效良好,安全性高,值得临床应用参考。Objective:To investigate the clinical efficacy and safety of platelet-rich plasma(PRP)combined with fractional laser and minoxidil in the treatment of androgenetic alopecia.Methods:Thirty patients with androgenetic alopecia were randomly divided into control group 1(n=10),control group 2(n=10),and study group(n=10).The control group 1 was treated with fractional laser irradiation and5%minoxidil topically,the control group 2 was treated with PRP injection alone,and the study group was treated with PRP injection combined with fractional laser irradiation and 5%minoxidil topically.The efficacy and safety were evaluated after 12 weeks of treatment.Results:After treatment,none of the three groups were cured.Treatment of control group 1 was markedly effective in 2 patients,effective in 4 patients,and ineffective in 4 patients.Treatment of control group 2 was markedly effective in3 patients,effective in 4 patients,and ineffective in 3 patients.And the treatment of study group was markedly effective in 8 patients,effective in 1 patient,and ineffective in 1 patient.Through rank-sum test,the effecacy of the study group was significantly better than that of the two control groups(P<0.05).After therapy,the proportion of vellus hair and single hair follicles of the three groups were significantly decreased(P<0.05),the hair density and hair shaft diameter were significantly increased(P<0.05),and those trichoscopy indexes of the study group were significantly better than those of control groups(P<0.05).The scores of greasiness,pruritus,dandruff,and hair loss in the study group were significantly lower than those before treatment(P<0.05).There were no serious adverse reactions in the three groups during the treatment.Conclusion:The combination of platelet-rich plasma,fractional laser,and minoxidil in the treatment of androgenetic alopecia showed great curative effect with high safety,which is worthy of clinical application.
